Default Wallet Medical Info Card

I have tried to keep up with my Medical Info on one of the cardboard wallet cards I got from my doctor, but am constantly erasing to change something, or completely re-writing the entire card. I have trouble with my hands, and the small space to write makes it even more difficult. And, since my husband likes to keep a copy of my medical info in his wallet, it is double duty.

So, I finally did something I have been meaning to do. I created my own, and am sharing here for anyone who might find it useful:

It is in pdf format with fillable fields. I have enabled user rights, so even if you are only using the free Acrobat Reader, you can still save it to your computer. Then, when something changes, you only need to change that one item and re-print. You can also print multiple copies without having to completely write one or more sets.

It is designed to be cut-out and folded in quarters, making it the size of a credit card. Once folded, it is almost as sturdy as the cardboard cards, and since it can be reprinted over and over, sturdiness isn't really an issue.
