Default Any Kentucky painies in here?

I am trying to locate a former pain doc who treated me back in the 90s as I need my records from him. I kept them but they somehow ended up being stored in my daughter's garage. They're gone for all practical purposes. This chap's primary office was in Lexington but had several sattelite offices in Central KY. PM me if you might be of some help. Thanks.
