I think there may be an error/mixup/confusion here someplace -- not assigning it anywhere.

We are blessed with opiate receptors in our brains that allow our bodies to respond to opioid pain medications. The majority of these receptors are classified a[s] mu and delta receptors and a smaller percentage are NMDA. Nociceptive pain is primarily mediated by the mu receptors and neuropathic pain by delta and NMDA receptors; morphine binds to mu receptors only, while methadone binds to mu, delta and NMDA. Morphine does an excellent job of treating many types of pain, but because of its ability to bind to 100% of opiate receptors, methadone may do it even better.

Mode of action
Methadone acts as an agonist primarily at mu receptors, but also has affinity for delta receptors
and a weak affinity for kappa receptors. In addition, methadone is a potent N methyl D aspartate
(NMDA) receptor antagonist, a property not shared by other strong opioids.

Hello Pebblebeach,

I have been on methadone programme for chronic pain for 5 years. I currently take 20 mg 3 times a day (60mg) and find it to date the better med for me in terms of taking edge of pain with few side effects ie - im not drugged up. I had been on everything available and nothing worked as well or it left me drugged up, horrid side effects etc...
Only thing i find is that due to its half life i still get lots of breakthrough pain so need a fast acting opiate - i take oxycondone... i swear by it. I take in between 60-100mg daily. In Uk as these are classed as controlled drugs i get regular monitoring bloods etc... and dosages reviews. In UK we have to go into hospital to be tritrated onto methadone under medical supervision - please be careful with just a prescription. Make sure you start with low low dose.... thats quite disconcerting.
If you want to discuss further contact me
I hope and pray you find some relief from your pain
God bless x
