I am not really up on the latest in myelin repair research, but I think there is some evidence out there that the myelin can heal itself at least to some degree. People will report lesions "getting better" on their MRIs sometimes, but I personally question whether that is actual repair happening or if it is just a change in the clarity of the spot on the MRI. I don't know the answer at all and am open to being educated by others who may know more about it than I do.

The only real way to tell anything about the age of the lesion is by whether it is bright or dark and whether it enhances when they inject you with gadolinium contrast. Old lesions where the nerve tissue exposed by myelin damage has basically be killed off will sometimes show as a dark spot, some refer to them as "black holes". New, active lesions where damage is ongoing will show up very bright in the scans after contrast is added. Otherwise, if you are like me and all of the lesions are still hyperintense, with no enhancement under contrast, there is no real way to know if they are a few months or a few years old, unless you have had previous MRIs that would show the changes.
