Found a new app today, that actually seems really cool and you can link the account to different devices. But it's kind of a symptom journal that is supposed to help offer some insight into things that may set off various symptoms, such as your moods and the weather. It also has little fact cards that seem to lead to different resources and such. Seems kind of neat, and possibly something to make visits to the neuro a little easier as there will be a log, if you use it regularly, to track symptoms and help doctors see where you're normally at versus the tiny window they get during a semi-annual visit.

Anyways it's called MSself, it's in the App Store for apple, but not sure though I would think it's available for google play store.
