Default Rsd/crps

I have been out on disability for RSD/CRPS, bilaterial DeQuervains and CTS releases for almost 14 years. Yesterday went to my doc who has now told me he is NOT CERTIFIED to fill out disability forms. He has had me go to my chiropractor to manage my pain and flare ups for past 3 years. Why would this happen after he has filled out forms in the past. He has told me he will not do surgery on the pinched ulnar nerves in both arms as it would exaserbate my RSD symptoms. What is the reasoning for this? I thought he was my treating doc? One of the other docs in the practice did the Bilateral DeQuervain's surgery and I was then referred to him as he is the upper extremities specialist. Please give me advice.
