Default Newbie: HELP WITH SSA 455

1. I received the ssa 455 form. Is a handwritten form machine read or should I fill out a form online or scan mine in?

2. I had gotten advice to attach ALL my appts (it's a lot) but from what I'm reading (which isn't easy due to my vision), I should limit to space provided & not fill out the last item with extra detail or attach anything. Correct?

3. My dx has severely deteriorated into a related dx that qualifies for the compassionate care allowance. From what I understand, I should not change the dx (I still meet criteria for my original dx) or add new dxs. Correct?

4. If I'm Correct on #3 above, my original dx goes by several names & I cannot find anywhere the papers that were used for my original application for reasons I don't want to distract you with.

5. Pls reply ASAP, as the form was back dated by the time I got it and they said I can turn it in this week. Thank you.

I hope so. I need to finish & was looking for my original application and hit my head bad. If I didn't need ssdi, I would be living life not searching for stuff cuz I've been everywhere trying to save my life & can't find anything cuz I'm
Too sick & so I hit my head and trying to decide if I can go to ER and risk losing income. Sorry I'm unusually frustrated.

Apparently, if it's your first mailing, you actually have 90 days before they send u a second mailing (according to ssa). Item 5 is still my main problem. I need an example because my condition (for which I was approved) Doesn't have an ssa dx code, but its subdiagnoses do. So, I don't know if I should list the overarching term for my condition (my original app did list a non ssa dx term- I found a near final draft), or symptoms (as in the intructions they give examples like "BAD BACK," which I find hard to believe is sufficient, or list like medication management or ongoing care, complications, home health etc. has anyone successfully filled out this form?
