Were you essentially drug naive (no levodopa or agonist) prior to starting?

I have heard that the formulation runs around $500 a month.

Yes, that's true. I've never taken a prescription form of L-Dopa or an agonist.

$500 a month is quite possible. I spend that much myself but I need a high dose/
