Parkinson's Disease Foundation (PDF)...Survey
PDF has a survey up on their website. It took me about two minutes to take the survey. Go to www.pdf.org and see the banner at the very top of the homepage.
"This survey is intended to provide the Parkinson's Disease Foundation with more information on our website users and to direct us on how to better serve our community. Responses are anonymous and will not be shared with other parties."
