Default FYI on latest interpretation of ACA

This can't be good: as I understand it, under the new ACA (affordable care act, what a misnomer that is turning out to be...), insurance companies may be able to remove the cap on out-of-pocket costs (which leaves one wondering what exactly it is that the insurance company does cover)...here's the link:

I worry how this will play out for those of us that are dealing with chronic illnesses. It's little comfort that you cannot be denied health insurance coverage for a pre-existing condition if you have to pay virtually all of the out of pocket costs associated with it, or the annual cap on those costs is so high it'll bankrupt you.

Will those with long-term care/disability insurance fare any better? How will those particular insurance companies handle the higher, or worse, unlimited, cap on out of pockets? I can't see those companies happily forking over the extra millions that could cost them....
