Default planning for the future.

Have any of you dealt with the issue of planning for the future in terms of your care long term? specifically dealing with the issue of assets and marital status. My girlfriend and I are planning a wedding next year. The thought came to me that when it comes time to place me in a nursing home or other facility that I may be able to qualify for care as an individual but not when we are married. That is something else, huh? My first wife left me and now I have someone who I treasure. Yet it seems as though this is discouraging marriage. On the other hand, I know that I am under the protective hand of a loving God and he will guide my care! Just a thought.
