I can not open the full article, but the summary explains it:

Quote:
Conclusions

DBS + ODT in early PD may reduce the risk of clinically important worsening. These findings further confirm the need to determine if DBS + ODT is superior to medical therapy for managing symptoms, reducing the complications of medications, and improving quality of life. The FDA has approved the conduct of a large-scale, pivotal clinical trial of DBS in early stage PD.
DBS = Deep Brain Stimulation
ODT = optimal drug therapy

20 years ago it was thought DBS was only to be used in late stages, when medication no longer helps. Today the thought is it is beneficial earlier in the disease.

I'm finding exercise and ODT is working great for me. DBS is a possible option in a few years, but I'm cautious and DBS isn't a cake walk as complications due occur. If it gains me another 10-15 productive years, I would be game for it.

Default DBS docs in Europe stimulate different part of the brain than US docs

at 12.12 into the video " If you overstimulate the separation panic system it decreases the enthusiasm for life...as this attempts to summarize.... and if you could elevate the seeking mood/directly/the enthusiasm mood: it should have antidepressent effects, and lo and behold they published this paper...."

It appears that PWP were 'normalized" by restoring (albeit arbitrarily) the will to live?
