Default Parkinson's Business Cards

Weird, but easy question!

My friend has Huntington's Disease and I just now saw that she has a "business card" that reads as follows:

I HAVE HUNTINGTON'S DISEASE

It affects my speech and the way I walk.
I have difficulty controlling the
movements of my body.
I AM NOT DRUNK!

Phone: 630-443-9876

(On the back side of the business card you list your personal information ... i.e. name, phone, contact person, etc.)

Does anyone know if there is such a thing for Parkinson's?
