Book assisted care facility rating tool/website

The website below has a search engine built in so you can look up assisted care facilities (and nursing homes, etc) to see how they are "rated."

I am using it to find a place for my dad, who has Alzheimer's. I have found 4 good places close to my home which I will visit tomorrow.
