[Gastric acid secretion and absorption of levodopa in patients with Parkinson's
disease--the effect of supplement therapy to gastric acid]

[Article in Japanese]

Yazawa I, Terao Y, Sai I, Hashimoto K, Sakuta M.

Department of Neurology, Japanese Red Cross Medical Center.

Since an oral regimen of levodopa has been instituted for treatment of
Parkinson's disease, its absorption and metabolism has been well demonstrated.
However, its chemical characteristics of high solubility in acid solution and low
solubility in water have not been well known. We paid attention to this
characteristic and studied the relationship between its absorption and gastric
acid secretion in 38 patients with Parkinson's disease who became refractory to
therapy of levodopa. We measured the pH and amount of collected fasting gastric
juice. Gastric acid secretion was decreased in 22 patients (58%). In ten of these
22 patients, 30 ml of lemon juice was prescribed in every administration of
levodopa as a supplement to gastric acid for two weeks. Increases of L-dopa
concentration after 60 min. and 180 min. were observed after lemon juice
supplement therapy. Among the Parkinson symptoms, rigidity, akinesia, and small
step gait were improved in every case except one patient who showed decrease of
L-dopa concentration at 180 minutes. However, improvement of tremor was less
remarkable. We consider this supplement therapy to gastric acid is one of the
effective and useful methods in the management of Parkinson's disease.

How might this be related to the vinegar/bloood glucose situation? I have been randomly sipping a weak vinegar water drink throughout the day and have been less diskinetic than usual.

I'm getting lemons today. Wondering if to do both would be overkill>? Whatcha think Rosie? Ibby

My mom is 84 and has had Parkinson's for approx. 13 years. She currently takes 2- 25/100 Sinemet CR 5-6 times a day depending on when she gets up.
And recently her mds added 1-25/100 regular sinemet 3x a day. So she ranges from 1300-1500 per day. The only other thing she takes is an occasional Benedryl at bed time.
In the last month or so she has a big drop in blood pressure causing black outs/ dizzy. Took her yesterday to md and also called mds, both agreed that she should increase her fluid intake, use more salt, and wear compression hose. Her bp was up this afternoon.
Also just found out that her MDS is moving to Virginia, its always something!

I take 3 tabs a day, so 300. I also take stalevo, 3/day which has carbidopa/levadopa and entacapone in it. Not sure the ratio on that.

Default I'm another of the ...

600mg crew combined with 6mg pramipexole.

The Pramipexole dose exceeds the max recommended but it gives me good results, cuts down the l-dopa I need and hence results in less dyskinesia, (or so the theory goes).

I was dx'ed 8 years ago.

Neil.

Gee... do I get a prize for the most? I take 1 and 1/2 of 25/100 every 2- 3 hours including when I wake up at night for an average of 15 pills per day.
Yep I get all that great extra movement like dancing with the stars except that it ain't no tango. Really wish I could cut down. Additionally I take 1.5 mg Mirapex ER twice a day. The dyskinesia is killing me. I also get horrendous foot cramps that bend my foot in two and feel like the bones are breaking. Anyone have a magic bullet for dyskinesia?????
