Default 1st appt w Neurologist. Have questions

I finally have an appt. My Dr thinks I have peripheral neuropathy. I was wandering,when I fill out the new patient info,do I list every single thing I have,such as endometriosis,PCOS,depression,etc. Will the dr send my complete record to him or just what he needs to know about why I'm seeing him. I haven't had an MRI on my back since 2010 so do you think it would benefit me to get one and take it because I will telling him abt my back issues,sciatica,nerve pain,etc. My insurance ends the last day of this month so if I need to do something I need to get it now.

Welcome to Neruo Talk. Hope you get alot of information here. Instead of writing down all my medical conditions at the office, I type them out on the PC, then have the office worker attach it to my forms. This saves time, and I can be thorough. You doctors, in other fields can transfer records that are needed. Write down all your questions as well, you won't forget anything that way. I wish you all the best for your appointment. ginnie
