After 4 months of treatment can it still be "only" B12? Pins and needles,tachycardia

I checked the B12 absorption and it's regular. It looks like I'm slowly improving, but the pins and needles are harder to get rid of.
Now a good doctor (contacted through internet) is insisting that according to her most of the symptoms left are due to a possible anxious state: she says she is 100% sure about that, but it's difficult to believe
