Default Could this be Mononeuritis Multiplex?

Sorry to post again so soon - and I have posted this on the Sjogrens Workd Forum too - but as some of you know I'm seeing my neurologist next week and have been focussing on symptoms and questions for her. While this has been preoccupying me - I have suddenly twigged that I may have this mononeuritis multiplex going on rather than simply a "presumed" small fibre neuropathy.

To my surprise, my fingertips started itching and stinging badly a few weeks ago on both hands. I posted about a neuropathic itch here. I already had noticed a big escalation in pins and needles in both hands, starting in fourth and fifth finger knuckles. When this started, months ago now, I assumed it was the all part of the Small Fibre Neuropathy and that this was a Sjogrens flare up. It is also affecting the area around both eyes in the form of numbness now too - and has been affecting feet and legs for 3 years or more. I assumed it was RA related as did two neurologists - so I've been passed back and forth for ages now. I have had a sharp pain if I press the inside of my elbows for years but this has also suddenly worsened - especially in my worst affected right arm.

Anyhow now I have a confirmed diagnosis of Sjogrens Syndrome - thought to be my primary disease - I feel a bit alarmed by this neuropathy ramping up so dramatically in both arms and waking me in excruciating pain during every night when I try to move my pillo or duvet and 4th and 5th knuckles are on fire in both hands. The skin on both palms has rapidly become thick and is turning s yellow under my pinky finger. Could this be mononeuritis multiplex now I wonder?

I think it wil have to take precedence over everything else with the neurologist next week if so!

My understanding is that mononeuritis multiplex can be associated with, among other things, RA and Sjögren syndrome.

This fairly detailed information about it might help when you see your neurologist Mononeuritis Multiplex: Background, Pathophysiology and Etiology, Epidemiology.

Default Although--

--what is usually referred to as mononeuritis multiplex involves disruption along several unrelated, but distinct, nerve tracts.

A mononeuritis multiplex might involve the long thoracic nerve, and the perineal nerve of the leg, and the medial nerve of the arm, at the same time, but symptoms would be limited to the distributions of those nerves.

What you are experiencing seems to be more diverse and systemic, so it may well be a non-length dependent small fiber neuropathy/neuronopathy/ganglioneuritis, which is common with Sjogren's.

Have you ever had EMG and/or NCS studies?
