I’ve had peripheral neuropathy for 20 years and have never had a EMG test. I’m also type 2 and the PN is a side effect of it. My doctor just treats the pain with Gabapentin. Is there a benefit to having the EMG test?

--that you've had neuropathy for that long and you have not had such a test. A nerve conduction study (NCV) and EMG is usually a pretty standard test in attempting to ascertain neuropathy, even if the suspected neuropathy is small-fiber in nature (that is, affecting the non-myelinated nerves that affect perception of pain and temperature); the NCV/EMG tests cannot reveal small fiber neuropathies but can measure problems with the larger, myelinated nerves that subsume the sensations of position sense, vibration, mechanical touch, and all muscle function.

Though many diabetic neuropathies are predominantly small fiber, they can involve larger fibers over time, so it might be worth it to get those tests to see if the neuropathic process has progressed. You do say it's been many years.

How was this neuropathy diagnosed? Or it is just assumed to be a consequence of your type II diabetes?

For diagnosis my doctor ask questions about symptoms, used monofilament and vibration fork during examination.
