Default Can Vasculitis cause Small Fibre Neuropathy

Hi there Neurotalk people. Iíve only just discovered the Tapatalk app on my iPad so Iím sorry if there were messages and replies for me last year which I failed to respond to. Iím a bit of a dunce about technology!

Iíve just posted this on the SjŲgrenís World forum but thought I should post about this subject here too, having been told by someone on good authority, that some forms of Vasculitis can cause SFN. He explained the same thing as my vascular dr - but said it was the other way round in terms of which condition is causing what symptoms.

So my vascular dr says that the damaged small nerve fibres of SjŲgrenís related SFN are causing a vascular response in the small blood vessels. But the Vasculitis man explained that, with these small spots under the skin, it could well be the other way round. Only he says I would need a Vasculitis expert to untangle this and diagnose accordingly. Thatís not so easy for me to organise due to my Scottish location.

So to explain; I have had some very odd petechiae on my feet coinciding with feeling awful and blood in my pee. And my all body SFN has been flaring.

Iím seeing dermatologist, rheumatologist and then neurologist later this month so Iím wondering how many of us with all body SFN might actually have a form of small vessel vasculitis which doctors arenít identifying?

And does anyone know which condition the focus of diagnosis should be on for biopsy?

My only input into this is that I have a pretty severe case of "Exercise-Induced Vaculitis" or "Golfer's Vasculitis." It's supposedly a benign condition, but I can tell you it's caused some permanent discoloration and damage to the small surface capillaries in my legs. When I'd walk the golf in hot weather, the back of my calves would turn beat red like they were sunburned, but there was texture to it as the capillaries were inflamed.

Idiopathic, Full Body, Small Fiber Neuropathy since 11/2013. Symptoms include altered sensation in toes; constant tingling/vibrations in feet and lower legs; intermittent tingling in temples, scalp and face; full body "buzzing" in the morning; referred sensation in feet cause by touch; occasional bouts with severe cramping in my feet and legs; when flaring, full body burning.

Metabolic issues: FBG -88 A1C - 5.1 with glucose spikes near 300, can't metabolize alcohol, high LDL.

Current treatments:
  • Omega 3's - Fish Oil (2400 mg EPA, 1200mg DHA), 200 mg Magnesium Threonate, NAD+
  • 5 mg Meribin Biotin for low-normal biotinidase levels.
  • Low carb "back to the farm" diet, lots of water
  • Quell TENS unit, 5% lidocaine patches, magnesium cream, CBD oil

Thanks. I looked this up on DermNet NZ yesterday - although I havenít done extra exercise and do have a systemic disease. The rash is completely different looking. Mine is dark red/ purple spots under the skin ie non blanching. It did arrive following recurring UTI and bacterial infections and 3 courses of antibiotics and I do have raised IgA in blood so the HSP Vasculitis tarries more although only 1.8 adults in 100,000 are affected so getting it diagnosed will be hard I suspect.
