Default The cold or hot is difficult to address, but the nails?

Yes your nails likely feel like they are two sizes two small? Yep it happens....just be sure either you or a professional can trim them short to help that aspect. I say Professional as we can't always 'feel' and can cut ourselves and make things worse. I would rather be chicken than hospitalized w/an infection?
As for the cold or hot? AND both can and do occur...for cold, those barley filled heat-em pods are good for slow gentile heat [providing you don't really HEAT them, just warm them] for heat? I use a spray bottle filled w/room temp water on hot spots and let it evaporate.
Both feelings HOT or Cold are Very Intense!
Bon-Don IF your fatigue is not due to diabetes? Get yourself truly checked out by a neurologist regarding your fatigue....it's common among other neuro issues and, if treated promptly, can get things on the mend.
