tooth extraction dilemma ....
Hi this started about a month ago. I had a toothache went to the dentist ended up having four fillings. Two to start then the tooth behind started aching so did two more.
This is on the lower right jaw. The second tooth from the back would not freeze to have the filling done so they kept shooting it with freezing sorry I don't know the true name but it is like a bullet the dentist puts the freezing directly into the tooth. It took a few shots to get the filling done this type of freezing does not last long and so needs to be repeated often. I had the filling about a wk and it would not stop hurting ear jaw everything hurt back I went and booked a root canal for the next day. Problem is the tooth would not freeze. This was a different dentist at the same office this one said she was doing the bullet method but it did not feel the same to me as there was no pop or jolt like there had been previously. She must have given me 10 or 12 shots it just would not freeze she may as well have been giving me shots of water. Some places were freezing minimally none of these places were where we wanted the freezing to be. Left side of my temple for instance back of my head felt weird but the tooth did not freeze. She tried to drill it several times I could not stand it and even her putting the filling back killed me. She sent me to a surgeon who removed the tooth under sedation. That went great it was five days ago. I was given pain killers which are almost gone but the pain is not gone and the gum on the inside of my mouth had a white spot on it since the tooth was taken out it is getting bigger. I called the surgeon only to learn he is on holidays for a wk they will not give me any more pain meds and there is nobody to look at my tooth. They suggested I go back to my dentist if it did not settle in a couple of days and to rinse it with salt water in the syringe starting yesterday tho it was a day earlier than the paper instructions stated. I started the syringing but went to the dentist anyway the same day. He says the white bit which is getting bigger with my syringing is bone??? he was very busy and did not elaborate on this in any way. I do get the impression he is tired of seeing me. Problem is the white bit is getting bigger and bigger I will be out of pain pills today. I am thinking if this keeps going like it is I am going to have a dry socket which I hear if painful but doubt it could get much more painful than it has been. So what is the next step go to the hospital emergency is all I can think of doing. If there is anyone out there with any insight into this issue please add your thoughts. ps I had an accident 20+ years ago where I had multiple fractures of my jaw some simple some compounded this all healed well enough. Freezing for years with a different dentist was never an issue. Not sure if this has anything to do with this new problem or not just adding for long shot sake. |

For those reading this and wondering what freezing is..... Freezing is just another term for "anesthetic". The white spot at the site of the extraction may be a bony fragment which may or may not heal on it's own.... or it could be that the jawbone is actually exposed. If you are in alot of pain, not just achey or uncomfortable, then you need to be seen by another dentist to determine if you have a dry socket. If there is no one for you to go to, then your only other option is the hospital. The normal time frame for pain after a single tooth extraction is about 2-3 days. If constant pain persists after that time or gets worse, then that is not normal and the site needs to be evaluated. It is important to heal properly or you could end up with an infection in the bone. Please keep us posted on how you are doing. Bryanna Quote:

The part of the jaw where this tooth is was once free floating as it was broke on either side of this site I know this side of my jaw did not heal well from an old fracture. I think this bit of jaw bone between these two fractures has turned in towards the inside of my mouth just a bit. I am so curious as to why this tooth would not freeze was it maybe infected and did not show on xray? or some other reason? I had two root canals at this same dentist last year and neither of them froze completely I went thru it somehow crying most of the time. I told him this time if it happened again I was getting a new dentist. Tho this office it is a different dentist all the time as they are huge. Four years ago I was quite ill from I think an antidepressant Effexor xr I quit taking it cold turkey and went thru very bad withdrawal. I have learned that these type of drugs affect you bones and some people on the internet think they affect your teeth too. I am curious if you have heard of this type of medication affecting teeth? If this has nothing to do with it could you tell me what causes teeth not to freeze on a continual basis? What can I do to be proactive in the future I am getting afraid to go to the dentist. My last extraction last year did not heal for three months. ??? |

The dental surgeons office has advised him to cut away a piece of the jaw bone that is exposed and has been since the day the tooth was removed. I was to the dentist on Monday all he said was that is bone. This office has different type of dentists I think some do root canals some don't the one I am seeing today did two root canals on me last year on teeth that would not completely freeze so we have a bad history. Wish me luck. There will be no vacation for me in the Canadian wilderness one must be fit to such things and I sadly am not. Been on a heating pad days now there is no hydro there for my heating pad or I might think about it. Happy long wkend to all. I will be home with the heating pad |

Well by now I hope you are feeling better. When the dentist tells you not to brush a surgical site, it is because the area is very inflammed and needs to calm down and/or he doesn't want the area disturbed just yet. However, it is still a good idea to rinse with warm salt water to help keep the area clean. Did he instruct you to do that? How are things going at this point? Bryanna Quote:

I also do hope that you are feeling better by now. I had a tooth extraction about three weeks ago, two teeth.
Here in Germany we usually get things like "Paroex gum" for gently rinsing the mouth after the extraction (a couple of hours later). Sage tea is also perfect. Never touch the fresh wound and do not rinse vigorously). The pain should be gone after a couple of days. I have to admit that I never would go on a holiday - not to speak of woods with no doctor around :rolleyes: - with such wounds and inflammations. I also skipped our summer holidays for fear that something bad might happen. All the best to you. KR Monica :) |

I am not happy with the care I received seems to me they should have sought out the reason the tooth was so painful and would not freeze. The site seems to be healing just very slowly the hole itself I mean the bone as I said above is more and more exposed every wk. Considering I had two dentists working on this issue do you not think one of them should have suspected an infection? They told me themselves there could be things going on that would not show on xray immediately as there was nothing on the xray according to both of them... just a swollen mouth and tons of pain.:mad: |
So now it is October while the pain in my tooth area is not as bad it is not gone. Sleep is becoming a huge issue as lack of it is causing me to be not happy. Update at the end of Aug a piece of bone the length of my tooth just came out on it's own. I thought good now it will finally heal. The extraction site did close but the pain did not go away just lessened. It is sensitive to hot and cold I still cannot stand anything in that area of my mouth. I went back to see the oral surgeon last wk and he took another xray said the bone is not filling in. He had no other suggestions other than to maybe come back in 6 months. I have been mulling this over for a wk today called the original dentist who could see me next Friday I nixed that idea as I figure why bother and told them I want a referral to another dental surgeon for a second opinion. I got that grudgingly the other surgeon has no appointments until November.
I am not sure what I am up against here but do wonder why this is such an issue. I talked to the dentist today on the phone told him the entire story again. He said the original dental work filling of four teeth (when they forgot to add the metal mould thing till after the filling had partially hardened) was not an issue. At the time of the original fillings that tooth would not take normal freezing and he had to keep returning to the room and doing a shot type freezing into the tooth. When they attempted a root canal on that same tooth a couple wks later that dentist tried to do that shot freezing it did not feel the same and it did not work. She tried a dozen times to freeze it and nothing on the inside would take the freezing this is where the bone was that eventually worked its way out. When they took the xray last wk I asked if they would be able to see if there was another piece of bone left in there they said it would be hard to find because it would be bone on bone and may not show up. Today I am thinking maybe I should try my doctor and see if she could get a cat scan of this area. This is four months I can't believe it. I feel like they are not believing me maybe because I wait so long in between visits and complaining. I have been complaining a lot to the receptionist at the dental surgeon she keeps telling me it should settle down soon. Also they give not appointments for follow up so you have to go there between 8-10:30 am and wait. The waiting room has no air I seem to get a head ache as soon as I walk in... I have had two consults after the extraction and both times waited the 2hours plus.. really a deterrent when they don't do anything anyway just offer more pain pills which make me feel weird and sick and I don't want. I am beyond myself with this now I am thinking I may have better luck with my gp and a cat scan. Does anyone here have any advice on how to proceed with this or suggestion of what could be causing the ongoing issues? |

This last time I had been on antibiotics twice and it was not going way... so went back did the xray routine again. This time there was a tiny bump at the top of the gum I would not leave till he looked... he did not know what it was and sent me to a specialist. The specialist said it was one maybe two teeth that needed the root canal redone... maybe three. He does not take my insurance which does not cover that anyway... so back to the dentist to find a specialist that will take my insurance. I find him but can't get in for a long time... I am on my fought antibiotic before the apt comes and it is not working so I call... have to go to the other speicalist pick get the xray the first dentist sent... take he looks ...uses the drug suggested by the drug allergy testing place the tooth will not freeze get another shot it falls out easy as pie ... he said it was a bad tooth he also said we will start with that one and keep going if we need to. It was easy. He did one pull it came out did not strain my neck or shoulder will be important infor soon I go down in the ellavator and my shoulder feels odd feel like I may vomit but don't at home my shoulder is very painful and I take codeine sleep. I was just finishing my antibiotic at this time... in the morning my back hurts so bad I can barely walk. The next day it is eased... that was Wed on Thurs I can't get up.. my back hurts in a different place I go to the clinic am told kidney infection.. get macrobid go home.. Friday I vomit all day.. Sat felt great when I first got up but it didn't last... sat in the night my face on that side swells I go to emerg in the night by the time I get in the swellling is gone they say finish the macrobid I will be fine but I don't feel fine.. I finish the macrobid go back to the clinic get another urine test and more codeine... doc says may be a stone go to emerg as they can test faster if it doesn't get better... next night I am in emerg... tell my long story get a different antibiotic cephalex and morphine ...but I don't take stay with the T3 I know as new drugs scare me. Finish that drug go back to the dentist the one who pulled the tooth in the mean time...before the antibiotics are gone.. he doesn't recall .. I am so sick I fall asleep waiting on him and am stupid when he come in. My xray has been sent back to the first dentist who took it he asks is there pain I say not in the tooth just a pulling feeling as both teeth left there have root canals I don't feel much but my jaw and my foot kill.. told him what went on since the tooth was pulled. nothing... He goes out says no sign of infection end of visit.. I am still waking up. Finish that antibiotic go back to the clinic told I need to see my own doc she no more antibiotics but she gives me more pain pills. She says I need to get over this antibiotic thing as I have had six now...ok I try. I resolve to wait as there is not infection in my blood on the cbc but the emerg doc said I have been on so many antibiotics it could still be there and not showing cause of pills so I am determind to wait htis out and if there is anything there it can grow be identified and killed... I don't make it. When I first had the sinus infection maybe wk 2-3 I woke with a pain in the side of my head like I had been hit with a bat it stayed a wk but eased and left my doc thought arteritis and sent me for blood work and to the eye doctor... Both dental specialists said that could be the infected tooth. My doc kept giving me antibiotics to keep meningitis away as she was afraid of that.. so am I. A couple days ago I get that pain back in my head and my jaw is painful again.. my doctor is now back from vacation I have an apt for next wk but I call and whine and get in... again my lungs are full this goes with sinus... she give me a new drug ceftin... and says see the dentist...again I get the drugs and go to the dentist office the original one to talk to them she said she did not get a report from the specialist and will call and call me back this moring I have slept all day she did not call we are now into another wkend... this has been going on way too long. I feel spent... confused and that there must be a way to find out if this is tooth infection or not?! Something conclusive the problem is I do not want to wait to get real sick again and can't seem to get taken seriously unless I am half dead the tooth show no signs ... but something is not right... it did hurt this morning I took more T#3 at 4am.. Ideas advice please. thanks I am keeping this in one place hope that is not a bad idea. |

I am so sorry for all the hell you are going through. You are right your dentists and the er drs are not getting to the root of the problem. At this point you are very ill and not only from the "source" of the infection but from all of the medications. I am not a doctor so I can only give you my professional opinion based on my experiences and what you are telling me. Number one.... Any teeth that you have that are root canaled are diseased. Plain and simple they are sick and infected. It does not matter who did the root canals, how well or how crappy the procedure was done or if you have any obvious tooth related symptoms with these teeth. ALL root canaled teeth are chronically infected and it is common for the infection to proliferate to the bone, the sinus, the blood stream and beyond. Number two....There could be residual infectious bacteria in the areas where you had teeth extracted that had been previously root canaled. These infections do not always show symptoms in the area where the tooth use to be. The symptoms can be more "systemic" than tooth or mouth related. These infections are not always picked up on routine x-rays. Number three... If you have periodontal disease or rampant tooth decay these are all infectious diseases which would be adding to the overall burden of infection. Have you seen an ENT doctor? Perhaps a visit to your primary care doctor explaining your ordeal and ask for a recommendation to an infectious disease doctor. I wish I could offer you more help here. I hope you find relief soon. |

I am not sure why I expect so much from this modern medicine that we supposedly have but I thought there would be something they could do to figure this out. My doc has sent me back to the dentist who I did not connect with last wk ... did I say that already... sorry if I did I will call tomorrow. I think I am not thinking well... not right it may be a side effect from the pain pills and the antibiotics. I have pain in the strangest places... it is odd it moves around but always on one side seems it can pop up any place on one side. I am quite sure my doctor is not expecting or wanting to see me till these drugs are gone that is how it goes come back when your sick again. Thanks for answering me it gives me something to do to type this out while I am waiting for pills to kill this... any chance of these finally being the right pill and I just get better and stay that way? I have never heard of a infectious disease doctor so would not have asked to see such a person I will ask at my next apt if I make it that long going the way I am going. As for this " periodontal disease or rampant tooth decay " I don't have any cavities at all. I have always had top notch dental care tho I did break my jaw 30 years ago in a car accident and needed braces. Having the jaw wired caused gum recession which is costing me now. Looking back I think some dentist took advantage of my great dental insurance I do wonder as I have plenty of root canals and since my teeth have been a priority after early trauma I wanted to keep them healthy. Perhaps my idea of care and their idea of profit collides to my disadvantage as I had to trust them as I don't know.. they do. If I were to have periodontal disease would this not be something which be obvious to the least qualified of dentists? I am seeing my own dentist again this Thursday there is no obvious sign of any infection in my mouth and never has been much to see in there even when the infected tooth was still in there. It seems to me it should be but maybe I am wrong maybe it is something that does not show up easily. I just don't know. I have had receding gums since my mouth was wired shut and have taken care of them best I can. Due to the antibiotics nothing showed in my blood the last time they checked it. I was so sick the emerg doc gave me morphine and antibiotics yet nothing showed. I am thinking better today and that in itself is hopeful but it can plummet fast I have been thru this a few times now and can't believe how I can just wake up so much worse. I would like to take this opportunity to thank you for all the help you have given me already. I truly appreciate it as I tend to worry a lot about this when I am well enough to worry I am well now having a bit of advice on how to navigate this has helped a lot thank you. |
My jaw is broken
I have had ongoing issues with teeth...
Here is the routine I have sore gums feels like a tooth the teeth 3 in a row have root canals so they feel different than teeth without should they ache at all. I go to the dentist he takes an xray says it must be sinus I go to the doctor she gives me antibiotics nasal sprays containing steroids ...sometimes it has moved down to my chest and caused infection there. It goes around like this countless sinus lung infections. Then fallout from antibiotics repeatedly. In March this year I did this same dance with one difference when I went to the dentist I had been on 3 courses of antibiotics that had not worked so was double checking teeth. Did the routine xray which was again fine. There was a tiny lump at the jawline so had the dentist check it. He was not sure so referred me - to a specialist who did not take my insurance.. so then to another specialist who did take my insurance. I had been on antibiotics from March to June by the time the tooth was pulled. Early June I think the 4th. The tooth almost fell out the specialist said it was a bad tooth or it would not have come out so easy. I had a different freezing that had to be done twice as a non freezing experience was in progress again the freezing was the one recommended by the drug allergy speciaist stated with an M still same family as lidocaine. It was a piece of cake ... the dentist was in the room 2-3 min. and gone.. I looked at the tooth as the assistant said I could it had a ball of puss on the end. The teeth behind still hurt... went back to this same guy later he said they were fine. Did not xray. After this tooth was pulled withing 20 min had a pain in my shoulder that went down my back in a couple days later I was dx with a kidney infection and put on more drugs... when it looked it up it was for uti. After starting it I was very ill vomiting ect.. could not get up... next day felt better but my face swelled up.. went to emerg was sent home said I was being treated already let the med work. Went home finished med... severe back pain after med was gone went back to emerg... pain was in my spine... given different antibiotic and different pain meds went home slowly improved. I did not want this again it seemed to me a pattern of infections and pain.. I had been 7 antibiotics from March to July. Aug I was sick from antibiotics over load treated that and avoided antibiotic therapy till Oct.. even tho I was not well. Feel like I am getting it all again now. I pushed both my gp and dentist to find out why ... dentist finally said this is out of his field he can't do pathology and gave me a name of doc for my gp to refer me to. I finally got in Oct 14 it cost a lot of money for me... she said I had muscle spams in my jaw and took full mouth xray said it showed I had arthritis. Told me to try an muscle relaxant or accupuncture. That was a month ago I seen my doc Wed last wk.. got the muscle relaxant... they give me head aches even at a much lower dose than suggested... tapering off them now. On Thurs I had a call from my gp saying I they needed an ok for a ct scan of my jaw as the radiologist from the hosp where I seen the specialist who said I had arthritis sent a report on Thurs saying my jaw was fractured and there is a piece of bone loose in there. I am gobsmacked this one xray I could not seem to get until I travelled to another city... finally showed what may be my problem a broken jaw. MY doc was not in the office it was the nurse who called. I have not been able to talk to any doc as it is the wkend. I would like to know if a broken jaw that went undiagnosed for who knows how long could cause all these infections I am experiencing? I just don't know where to turn next the entire things is confusing to me. I have had to fight tooth and nail to get T3 from my doc so I can sleep at night all day I just put up with it. Some days are hell on wheels some days are better. I feel let down and not well cared for I also feel unheard .. I don't know what other people do when they are in pain but maybe I am not presenting in a way to get the attention I need. I don't scream and cry when I am in pain... not in front of people... I say I am in pain and expect them to believe me. That is the rant part. ? Can a broken jaw cause infection if it is let go indefinately? Will fixing the jaw stop the infections? This is what I really want to know secondary question how on earth did my jaw get broken but I doubt you can answer that. Thanks for your time. B |

I brush and floss daily even when it hurts I have an electric tooth brush that is supposedly good at beating plaque ... at a loss of what to do next .... I may have to wait lord up to 2wks or more for this ct scan....no idea where we go after that or how long it may take in the mean time I am getting sick again. grrr... I did ask my doc to send me to an ent doc she said she did not think it was needed at that time as I had been already nothing was found. Sometimes I feel like nobody hears a word I say and every doc is content to feed me antibiotics and pain pills to lecture me later on the over use of both... yet it is not affecting their lives just mine. pulled teeth
I have a question I just got 13 teeth pulled 9 days ago my face swelled real bad but went down yesterday which like I said made 9 days since my surgery I put up some Christmas lights out side this morning when I woke up one side of my face is swelled again should I be concerned

I am so sorry you are going through such hell. But please know that this post is not dead. I am the only one here who can offer you professional dental information. I am only a volunteer here so I fit this into my schedule when I can. Your case is very complicated for many reasons and one of them is because I think you have other dental issues that you are not disclosing here and/or that you are not aware of. The repeated antibiotics are going to make you seriously ill. It is a huge misconception that antibiotics can be the cure all for all infections. The only way for an antibiotic to get to the source of the infection is to first kill off all of the protective healthy bacteria and in your case any healthy bacteria that you had is long gone and been replaced with additional unhealthy bacteria. The antibiotic also has to be specifically sensitive to the species of bacteria that is causing the infection. Otherwise, it will not be effective. The only way to know the species of infectious bacteria is to have a sample taken of the bacteria at the site of the infection and have it cultured. I really don't know how to help you at this point. Early on I gave you suggestions and information based on the information that you had been providing here. I will re read all of your posts and see if I can offer you anything else. I really feel badly for you. But as long as you are willing to keep accepting the same protocol from your dentists, this situation will only get worse. Bryanna Quote:

I had a fractured jaw broken in several places in a car accident in 1985... I am sure I said all this but maybe I missed something or did not think it relevant to a tooth extraction. Even looking at my xray the pathologist at a major hosp did not see a problem except she said I had arthritis which i had been told before and which in my mind accounted for the jaw pain... she suggested muscle relaxants only. It was a month after seeing here that a report came from the radiologist at this major hosp saying I had a fracture. It may be that the fracture is difficult to detect that only a person with precise training was able to spot it... I am not mad at the people who did not see it not yet... at this point I am just glad somebody seen it and hopeful it can be repaired. I had no idea my issue was a serious as it now seems to be... I got use to the pain as thought it was arthritis but the infections were a problem I know the antibiotics are not good for me believe me I know better then you what they can and do damage in my body still when nothing else works they are the only game in town that I see just now. A bandaid for sure but one I can't live without as this affects my thinking and is very painful when the infection is there. I have been to emerg it got me no place... but sent back home on more antibiotics. I think it is best to follow up with people who are aware of my history then the hosp emegency room... where they are in a hurry and often just sent me home with pain pills. I am not happy about this either but unless you know some way to make people in health care do what you want ... I don't know what else to do. Thanks for taking the time to write I guess I was just hoping something would be obvious and it isn't simple, or common I guess. I have been stressed by the state of affairs for a long time and really don't appreciate you saying I am accepting... as I don't feel I have any choice at some points and have kept pushing to get this resolved or would not be where i am not... granted it is still not a good place to be... but better than not having any clue what is going on ...now there is a place to focus at least. Terribly sad it has taken so long I can't help it if people don't believe me pain is a very difficult thing to impress on others they either believe you or they don't. I can't make them... I am not a cryer I should not have to cry my words should be enough.... I feel like your blaming me in some way and that is unfair ... there said it straight up. I have used this site as a source of information hope and stress relief when i am feeling panicked but think this is the end of the posts from me as I feel insulted. I may or may not come back and tell you what happens it may depend or your response to this post or just my lack of humour over this entire situation. |

I sincerely hope that your venting makes you feel better. That is the strongest you have "sounded" in a long time and I am relieved to see that you still have some fight left in you! I sincerely hope that you can use that same tone with those dentists who are jerking you around. I know how difficult it is to have to demand that you get quality care especially when you don't feel well and you are in pain. I see it every single day with people pleading for answers only to be prescribed yet another medication. You can read about that very thing on this forum over and over again. I can only tell you that you have to keep trying and you have to stop accepting their lackadaisical diagnosis by prescribing you yet another antibiotic. Again, I know how difficult that is to do and please understand that when I say that to you it is not meant to insult you or to insinuate that you are to blame for anything. It is meant to keep the fire in your spirit to be a strong advocate for yourself. I hope I am making myself clear on that as I have no ill intent here what so ever. I have spent a lot of time here talking to you over the years.. I think that shows that I do care about what is happening to you. I looked back over all of your posts beginning in 2011. Your dental problems have been ongoing for a very long time and you have taken numerous prescriptions of antibiotics and pain meds. At this point your system is truly ill and none of your dentists, physicians, not even the oral pathologist seems to give a damn about your health. Am I right about that? We talked about your root canaled teeth and you said they ache all the time. I explained that irrelevant of what shows on the xray, those teeth are chronically infected and I explained why. Your dentist keeps ignoring those teeth and misdiagnosing them as sinus problems instead. It is typical for dentists to do that because they see nothing wrong with root canaled teeth. Keep in mind... dentists are educated to "retain" teeth no matter what. Root canals do not cure infected teeth and they know that but they will rarely share that with a patient. Root canals are done to attempt to buy time with infected teeth and that is all. Infected teeth can and do lead to sinus infections.. not uncommon, just often not correlated with the teeth. You mentioned having a kidney infection, again not uncommon for someone who has oral infections, just not often correlated with oral infections. You've had numerous visits to the hospital, physician, different dentists ..... my God you have truly been through hell. I want to address your broken jaw as I feel this may show yet another ill or misdiagnosis. The area that was recently diagnosed as broken could that possibly be a scar in the bone from your previous brake? Or could you have some form of osteonecrosis going on? The dentist who saw this should have compared the location of it to previous radio graphs taken at the time of your accident. Was that comparison done? Arthritis in the jaw joint can be painful and intermittent usually brought on by change of temperature or overuse of the jaw joint. Most often it is not a constant pain like infection would be. However, you may have some undiagnosed problems with your jaw, like osteonecrosis, or TMJ that may be related to your accident. Has any of that ever been brought up by your dentists? You could also be gridning or clenching your teeth which would cause your jaw to ache and cause pain in various areas of your mouth. Was that ever discussed? You had ortho when your jaw was broken in the accident and ended up with gum recession. Was your jaw wired shut or did you just have braces put on your teeth? Either way, it could be that your bite was never aligned properly. That too should be looked into by your dentists. I wish I could help you more than throwing this information out there to you. I can only offer this to you and hope that you are able to do something with it. Make a diary of everything from the accident to present. Write down all the things I have mentioned to you throughout our posts and make a list of everything you need to discuss with your dentist. Please don't settle for another medication....you have to pursue the diagnosis the best way that you can in spite of feeling lousy and in pain. I am so sorry you are going through this.
