root canal
I just register but read before through some posts and would like to ask for advice. I have constant problems with two root canalled teeth. I have some pressure on them and some kind of neuralgia and paresthesia on that side of my face. Always been told teeth are fine. At first it was four teeth with root canal, other dentist confirmed they are not good and did a surgery. Then he said two wasn't successful and pulled them out and the remaining two said it's fine. I have a bridge there. So long story short I have now that two and think they making me problem. Went to hospital because all the dentist have said everything fine. At hospital they did x-ray and confirmed one is not done correctly but the other one is fine. My wish is to remove them both because I am sick of it. I have them like this since 2009.
Sorry for my English, it's not my first language. I didn't say what teeth are problematic. I have a x-ray and can post it so someone can look it up. Thank you for any response. PS the teeth are upper left side I think that is 9 and 10 (first and second on the upper left side) |

Welcome Marija! I am in the dental field and can offer you some information here. Your English is fine :) Thank you for posting the x-rays. The film clearly shows that both of these teeth are infected and that the bacteria has spread to your jawbone. This is typically seen with root canaled teeth because there is no way to sterilize a tooth and make it healthy once it has become infected or once it has been root canaled. Very often a dentist will say that the procedure was not done correctly and needs to be redone or they suggest a surgery called an apicoectomy. Which is what you had done. Unfortunately, the fact is that how the procedure is done is irrelevant simply because the anatomy of the tooth does not allow access to the hundreds of tiny canals inside of the tooth which means these areas will always contain infected nerve tissue once the tooth has become infected or once the tooth has been root canaled. You also have posts in both of these teeth which causes further damage to the teeth as well as additional inflammation. Posts are put in to give these fragile teeth some anchorage for the bridge. However, they often cause the teeth to fracture and are a reservoir for the collection of bacteria. It also looks like you have piece of a broken file inside #9 which is not the cause of your problems it is just another source of inflammation. It is a shame that you have been through so much dentistry in an effort to retain these teeth. The truth is these teeth are irreversibly diseased and will never be healthy again. The longer they remain in your mouth the more widespread the bacteria and inflammation will become. Having the neurological and paresthesia symptoms clearly indicate that the bacteria and inflammation has migrated into areas that will cause you to have systemic/physical health issues related to these teeth. Replacement options for these teeth as well as the other two are limited due to the deterioration of the bone from the long term infection. Dental implants would be very risky in this area due the infection. Your only option may be a partial denture. You would need to discuss this with your dentist and an oral surgeon. I know this is scary and upsetting to think about but I urge you not to wait to take care of this infection. Please keep in touch here..... Bryanna Quote:

The oral surgeon that removes the teeth may prescribe an antibiotic either just prior to the surgery or at the time of the surgery. It is best to go for a consultation first and discuss everything at that time. Antibiotics cannot cure the infection that lives inside of the tiny canals inside of the teeth and the medication cannot cure the infection in the jaw bone as long as the teeth are present. The relief that is sometimes achieved from taking antibiotics for an infected tooth is temporary as the medication may temporarily subside some of the inflammation and bacteria, but that effect will be short lived as long as the infected teeth are present. Oral surgeons will generally not discourage someone from having root canaled teeth extracted. They are very aware of the chronic infection brewing inside of these teeth. It is important for you to understand that dentists are taught to "retain" teeth even when they are infected. That is why root canals are done because they give the opportunity to "retain" a diseased tooth for an undetermined amount of time. It is not a secret that a root canal procedure cannot cure the infection... but this fact is rarely shared with the patient and for many reasons. You will have to discuss the removable partial with your general dentist prior to the extractions so it can be made in time for the extraction date as it may be necessary to insert it right after the extractions. This is something your general dentist can have done for you at his dental lab after impressions of your mouth are taken. The partial denture would either be delivered to the oral surgeons office or you could pick it up at your dental office when it's ready. Sometimes, the oral surgeon prefers that the general dentist insert it... other times not. Again this needs to be discussed with both dentists. It is important to disclose your ill health with the oral surgeon and even if you feel that he is doubting the connection to your teeth... don't let that discourage you from expressing why you feel there is a connection. I went today to dentist. My pain in face is worse so I couldn't wait till Monday. I have been feeling ill for several days which I blamed on possible viral infection. No more waiting. I got antibiotic and dentist agreed one tooth is bad and he is willing to help me. I am having appointment on Monday to discuss everything with him. I told in short what I want and that I want both teeth out. So I hope that I will resolve my problem soon.
Thank you for your help. I found lot of good info here and read a lot how bad root canal teeth are. As a matter of fact several months ago I pulled out one I had. It didn't make me lot of trouble but I decided to get rid of them all. That one was easier but I waited a bit longer for this front ones cause of some financial problems and it looked a bit complicated with that bridge. I will post next week what I did at the dentist. Glad to hear that you went to the oral surgeon for a consultation. I think you know that it is inevitable that both of these teeth will need to be extracted. Retaining even one of them means keeping the infection as ALL root canaled teeth are chronically infected. So it is important to consider all the issues that are at stake here. I hope you and your dentist are able to mutually agree on what needs to be done so you are rid of the infection and will thoroughly discuss your replacement options before you do anything else. I have a problem with anaesthetics with adrenaline/epinephrine. I always ask for one without it. Will this affect extraction of root canal teeth? Also I am taking antibiotics and will that calm down infection before extraction or should I worry about some complications? What should I discuss with the oral surgeon before extraction? I was yesterday at the clinic and discussed everything with the surgeon. I have an appointment for Friday morning. The both teeth will be extracted as I asked for. The bridge will be cut so I can be left with one healthy tooth which was keeping the bridge on (no rc tooth) and the two problematic ones for which I posted the x-ray will be extracted. He wanted to leave one of rc teeth, claiming that is good but I was clear about me wanting to take out rc teeth for which I strongly believe are making me sick. I have a total of 5 teeth under the bridge, 2 missing teeth, 2 rc teeth and one healthy for keeping the bridge on (I hope I explained ok.) So the helthy one will stay under the crown. The Dr was nice and respectful for everything I ask for. No epi anaesthesia. Will do partial prosthesis. Everything will be ready on Friday morning. And he said he will 'scoop' after extraction if I understood well that expression. I am on Augmentin till Saturday evening. He said that when he removes the teeth infection will clear so AB's are not needed when I finish this round. Tank you for help. It means a lot. :circlelove: |

Okay, that sounds like a good plan to me! You have made a healthy choice to remove both teeth. Hopefully you will notice a positive improvement in your health very soon! It is best to prepare a nutritious diet plan and purchase what you need ahead of time so that you do not have to think about that during the healing phase. A nutritious soft food diet is recommended. Avoid eating things that are not healthy .... sugar, artificial sweeteners, soda, anything diet or low fat, refined foods like white breads/frozen meals/ canned foods/ pre made meals/fast food. It's a good idea to make a large pot of home made soup or stew, freeze it in small glass canning jars so you can just heat it up with little effort. I am taking antibiotics now but pain and neurological symptoms are present still after 3 days on AB. Maybe is still early. Dentist said that after the extraction infection will clear. Hi Marija,
The infection in the jaw bone is not always clinically or radio graphically visible unless it is large enough or widespread enough to be seen. Because the bone is so porous and because the mouth is so vascular, the bacteria can migrate to areas way beyond the teeth. Antibiotics will do the best they can at eradicating the bacteria but there is no guarantee the infection will be completely gone. This is why it is never a healthy option to retain an infected or rc tooth for a long period of time. A few things that do help in eradicating the infection are: For the surgeon to be very thorough in scraping out the bony socket after the tooth is removed. In doing so, the surgeon can feel with the instruments when he has reached solid bone. Also the color and amount of blood that comes from healthy bone as compared to unhealthy bone is quite different. So these are also indicators that he has either reached healthy bone or there is residual bacteria. A nutritious diet that is rich in whole fresh foods and low or zero refined and processed foods. Avoiding sugar and artificial sweeteners all all beneficial for our health. A healthy diet is key to keeping a healthy immune system. During times of stress, infection or illness the immune system is compromised and we can only expect it to take care of so much at one time. So eating healthy takes the burden off of the immune system so it can do it's job at fighting the bad guys. Take a probiotic supplement every day. This provides the immune system with healthy bacteria which helps it to work more efficiently. I am not surprised that you still have pain and neuro symptoms. Our teeth are intricately connected to so many nerve areas that branch out to so many other distant areas. While those teeth are still present, the antibiotics cannot cure the infection because the bacteria is still brewing inside of those tiny canals inside of the teeth where the medication is ineffective. So the meds are only going to temporarily subside the inflammation until the teeth are removed. I know you are scared and you are fed up with this pain. You have a ways to go before you feel better, but you will feel better. Removing those teeth will bring great relief to your immune system but in order for it to do its job well, you need to feed it well :) If you have any other questions, please feel free to ask me. I am having some low grade fever on and off during the day, today and yesterday. I've been feeling somehow sick since two days ago and the pain in still there, little nausea an general feel sick. Yesterday afternoon went to the dentist to see what is going on since I've have been using antibiotic since last Saturday and take some panadols and started ibuprofen since panadol doesn't work. He said he can just switch me to some other AB's and will do extraction on Friday (tomorrow). I am so scared that something is wrong. it affects my sinuses and it always does since the beginning and I have some nose bridge pain.

It's done, the teeth are extracted. I am at home and will take a rest. Still a bit emotional and having some pressure in face and anaesthesia still works. I will post a bit detailed later. I hope everything is going to be good.

Extraction was on Friday. I was very nervous and emotional. Surgeon cut through the bridge and extracted the teeth. He scraped the infection. I could feel that. I hope this is what was so important after the extraction. I feel some sinus pressure and sensations which I had as a ongoing problem even before the extraction. What worries me is low grade fever today. The same I had before extraction. The gums are sore and purple. I've just finished the antibiotics yesterday. I've got partial dentures and that is a struggle. Tomorrow going to the dentist to adjust dentures a bit. I hope everything is ok. I would like to attach some of my x-ray to check for the teeth next to the extraction teeth. My memory is not excellent so I don't know if I have more rc teeth. I know I've had one on the upper left side but I extracted it last year. The x-rays are before that extraction so on the x-rays it could be one rc tooth. I really don't want any rc teeth in my mouth. Soreness and discoloration of the gum are normal. The low grade fever can also be normal. Make sure to drink lots of water throughout the day as this will keep you hydrated, flush out the toxins and help reduce the fever. Thank you for posting the x-rays. It is difficult to tell which side of your mouth is shown in these xrays. On the second xray you do have another root canaled tooth on a pre molar which appears to have a silver point in one of the roots that has possibly perforated the root tip. This tooth is in direct relation with your sinuses. Whether the root or point has perforated the sinus that cannot be determined by this xray. There are also some distinct markings on the xrays that are either pieces of metal points or artifacts. I cannot tell which from these views. On the first xray the area in the bone where the previously extracted teeth were removed does not look healthy. Has anyone talk to you about that area of bone? First x-ray is my right side. 2 teeth missing since 2009. Those were also rc teeth from long time ago. No one told me bone is not healthy. Those were 8 and 7. Then on Friday I had an extraction of 9 and 10 for which I asked for help in my first post. So second x-ray is my left side after the 8 and 9 which I extracted on Friday. There was a rc tooth which I extracted last year ( I think number 13). This is a bit old x-ray.

Okay thanks for clarifying the sides. On the one xray that you have provided, the bone in the areas of what you are calling #7 and 8 has not filled in properly. It does not appear to be solid which would indicate a possible long term infection. Sometimes these infections are known as cavitations. These can occur from the infection in root canaled teeth, apicoectomy surgeries, or inadequate scraping of the bone when the teeth were extracted. An oral surgeon should take a new xray of that area and compare it to the one that you have to see if there has been any positive change in the bone. Bryanna Quote:

The quality of these xrays are not that great. All dental xrays are limited to only a 2 dimensional view. So the view is dependent upon the angle of the xray beam. However, it is fairly apparent on that xray that the bone in the area of #7 and 8 had not healed well at the time of that xray. A new one should be taken to compare. The problem with long term inflammation and infection in the jaw bone is that it can be very difficult to get rid of completely due to the abundance of blood vessels that transport the bacteria from one space to another. There are lots of tiny spaces in the bone for the bacteria to go into. Sometimes an oral surgeon will do some surgery to remove the bone that has deteriorated, other times that is not recommended. But even the surgery is no guarantee that the infection will be cured. It really depends on the individual case. There are no supplements that you can take to cure it. You can improve your immune system through proper nutrition, drinking lots of water, taking probiotics and supplements like vitamin C, vitamin D3, a good multi vitamin twice a day ... things like that. I am sorry to even share this information with you as I know it is very scary and I know you have not been feeling well. You need to see a very good oral surgeon who will thoroughly evaluate your dental health. If you could find one who has a dual degree meaning they are a dentist and also an MD, that might be helpful. I went to the OS who did the extraction because I started to have bad face pain and pressure, like a sinus pain. Generally feel sick and still have low grade fever on and off. OS said that I must be having sinus infection that has nothing to do with the extracted teeth. And sent me home. I went to general practitioner who then said it's not sinuses and it must be teeth. He said they were recently extracted and causing this face pain and pressure. He prescribe Augmentin. I am so desperate and don't feel well at all. I know I need more time after the extraction to maybe feel better. OS said he will do some xrays of my front theeth when I said that I am concern about my jaw bone. And he will do it in 6 weeks time to let the extraction site heal a bit. Marija |

I would suggest that you not wait the 6 weeks and see an different oral surgeon as xrays need to be taken of the 7 and 8 area to compare to the older one as that area of bone may be infected. If it is, then no amount of antibiotics is going to cure it. Repeating antibiotics is not wise unless there is a definitive diagnosis and reason to take them. Antibiotics can make you very sick and cause intestinal infection. Please see a different OS, one that is not associated with the one that you went to. Bryanna Quote:

Thank you for asking but I feel bad. I will stop the AB cause it seams there is no use. Pressure in face comes and goes, but good is that there is no pain in the extraction sites. Fever also still comes and goes during the day.I also have some other health issues so I do not know what is what. I have some b12 deficiency for which no one cares so I must start to fight for my health.

Unless you are having a reaction to the antibiotics, do not stop them abruptly. Antibiotics are only suppose to be used if absolutely necessary as they can cause serious intestinal problems. Your B vitamin deficiency indicates that you have an immune problem and faulty digestion. This needs to be corrected and it will need more than just taking a vitamin supplement. Okay, thanks for posting your panorex. Did the dentist take a full mouth series of separate xrays as well as this? Some dentists will automatically do that because this type of xray does not give as close up of a shot of the anatomy of the teeth as the separate ones do. This xray is okay for details about certain areas of the teeth, the bone, the sinus, etc. But the smaller xrays zoom in on the problem areas better. I will look at the xray and tell you what I see. Keep in mind that a closer view with those small xrays may give a clearer picture of some of these areas. But I will tell you what I see here. The xray is marked with an R for the right side and an L for the left side. To follow along correctly, when you hold up the xray, the R will be on your LEFT and the L will be on your right. Okay. Starting with the side marked with the R for your UPPER Right side continuing over to your UPPER Left side: #1 is a wisdom tooth that you are not using to chew with and it appears to have a cyst around the root structure. #2 is your second molar it appears to have severe recession and bone loss, deep pocketing and possible decay #3 a molar and 4 a pre molar are removed #5 is a pre molar with a possible fractured root and pathology (infection) in the bone. #6 is your canine it is not diagnostic on this xray. #7 a lateral, #8 a central, #9 a central, #10 a lateral are removed. #11 is a canine possible decay and is really not diagnostic on this xray. #12 is a pre molar possible decay and pathology in the bone, not real clear on this xray #13 pre molar previously root canaled is removed. The last molar on the upper left is either #14 or 15, one of them along with #16 has been removed. The remaining molar there has a lot of recession and possibly deep pocketing. On the lower jaw starting with the R for your LOWER Right side and continuing over to you L for the LOWER left side: #32 wisdom tooth is removed. #31 second molar has decay #30 first molar is root canaled, has decay and cystic activity in the bone. #29 pre molar looks okay #28 pre molar looks okay #27 canine appears to have decay on the root surface way below the gum line. #26 lateral okay #25 central okay #24 central okay #23 lateral okay #22 canine possible decay #21 pre molar okay #20 pre molar okay You are missing a molar in that quadrant and I think your other two molars have shifted forward. So lets say #19 the first molar is the one that was removed. #18 second molar decay #17 wisdom tooth decay. Regarding the bone areas in the upper front where teeth # 7,8,9,10 have been removed. There is a void in the bone in the area of 7 and 8. I'm not certain but there may also be a piece of root still in the area of #7. This needs to be evaluated. In the areas of 9 and 10 that bone looks like recent extractions. In the area of #13 your upper left premolar that was removed that bone has not filled in completely. How long ago was that removed? How does this list compare to your dentists findings? Bryanna |

I am shocked. I went to some private clinic for this xray. This is a new place for me. The dentist said everything is ok. I complained on some facial pain and my concerns about health of the bone after the front teeth extraction ( 2 several years ago and 2 just recently). He says everything looks ok. That is all he said. |
Just to add that this is the only xray I've done today. I didn't know what else to ask for.

You have some dental problems, everything is not okay... I'm sorry :( The small xrays that are used to take a full mouth series of xrays are called periapical and bitewing xrays. Not just bitewings as some offices due. There are about 18-20 small xrays in one full mouth series. They zoom in on the individual teeth and can detect decay much better than this panoramic view. I feel so bad that you keep getting jerked around by these dentists. Please don't stop here, you need to be evaluated better. Bryanna Quote:

I know that everything is not ok but I am not so good financially and I barely afforded my partial dentures for six upper teeth. What I can do from now on to start fixing one by one whit rc tooth being the first. Dental work is not covered with insurance here where I live, only extractions. If I extract my lower rc tooth will this affect much my partial? I just got that partial and I cannot change it now. I keep changing the dentist and I feel paranoid and stupid because every one of them saying that everything is ok. My daughter has 3 mercury filled teeth. Is it better to leave it like this than change something. I ask because here, all the dentist are the same and I highly doubt that anyone knows how to safely remove them. So maybe better to stay like that then to be even worse with mercury exposure. Also when can I start oil pulling after the extraction? Thank you Marija |

Please don't feel stupid. The fact that every dentist you have seen has not properly informed you of your dental problems is certainly not your fault. You are trusting them to be honest with you. I am not saying this is your particular case, but sometimes a patient gives the impression that they do not want to know the extent of the problems and they just want to deal with one particular area. When that is the case, more times than not the dentist will not inform the patient of all their problems. Sometimes people are not even aware they are giving the impression that they don't want to know everything. Could it be that you were so focused on the upper front teeth that you gave that impression? Even if that were the case, the dentist is legally suppose to tell you of their findings. To remove the lower molar will not interfere with the upper partial. Regarding your daughters mercury fillings..... do not remove them unless she can go to a dentist who follows the proper removal protocol. Not using the protocol is dangerous and whether immediate or not, the patient can become very ill. Are you in the US? If not what country? Bryanna Quote:

I would like to ask how to remove my last rc tooth which has mercury felling, in lower jaw? It looks like the filling is connected to the tooth next to it or that is just how I see on xray. How to safely remove that tooth? Thanks |
