SSDI CDR question
 
I have looked high and low and cannot find a list of codes for the SSA CDR= Scanline 2 –
Field 1 – Medical Diary Reason/Type

Does anyone have this list? Mine has a number (3) and I've read where it's typically a 3, 5 or 7, depending on how frequently they send out the CDRs....but my friend's has a letter (N)

Anyone know what the N means? I know my 3 means I'm sent a CDR every 3 years-ish.

yours is medical improvement possible 3 years to next cdr

i have never heard of a code N for that line and can find nothing on it. how old is your friend?

Thank you-- I knew what mine means-- my friend is in her mid 40s. I am 52. Mine was based on Depression/anxiety, hers similar, though also with additional physical issues.

L is low and 0010 is very low so you are very likely to get the short form in three years again.

I got my CDR and do I put my last 3 visits to my physicist since I see here every 8wks or even my primary cause i am a diabetic too. Do I need to put I had laparoscopic cholecystectomy done last July and was hospitalized cause I am confused since that has nothing to do with my mental disability. This has made me so stressed with more depression and axienty/panic attacks since I got this and had to schedule a sooner appointment then 8weeks.

I feel my disability got worse with axienty/panic attacks

for last 3 visits put down the visits that to do with the condition or conditions you were approved for. in the reason for visit put the condition or illness.

for hospitalizations put that hospital admittance for laparoscopic cholecystectomy and any other hospitalizations you had since last cdr

Thank you for that information. Why do I need to tell them about my surgery if that has nothing to do with my mental. Since I have became anti social,axienty panic attacks since covid and recently more depressed I am going to say worse. (Should I leave that reason in the remarks) But I am far from going back to work so my physicist and I have not talked about that. Should I still mark that box.

Don't leave anything in the remarks. if you leave the remarks blank, the form is processed by a computer program. If you write in the remarks a person has to review it.

it is ok to choose worse for that current state of your illness.

The reason i am telling to fill out the hospitalization is because that is what an official New York State guide to filling out cdr's said to do. They also said to leave remarks section blank. Also with hospitalizations it is asking for any hospitalizations.

Does that mean to leave blank even if I am in california? I am also worried cause we have not talk about going back to work due to I feel that's not something I am ready for yet. So if it's read by a computer won't it be red by a human if I am putting in hospitalization/surgery. This is so confusing

Thank you. So does anyone know why they want to know about hospitalization/surgery.? Is that to see if we are being honest or something? For my last visit should I put "follow-up,medicine,medicine " since all my physicist appointments are to come in and give medicine and see if we need to up or down them.

We have not discussed working because that has not even came up since I am not socializing with anyone(stay home in my room)

you are over thinking this and making it more complicated than it needs to be.

It doesnt matter if you are in california , new york or nebraska. the New York Guide was useful to everyone getting a cdr.

The only section you should leave blank is the remarks or comments section. There is no upside to making remarks or comments for a short form cdr.

Everything else you have to answer.

For whether you discussed with your doctor going back to work, just answer you did not discuss going back to work with your doctor. thats fine.

for the last 3 visits it is recommended to just put the condition or illness or illnesses that got you approved for ssdi or ssi. nothing else.

as far as hospitalizations, just put them down. there is no downside to that.

this is the best that i can explain it

Thank you for all this information. I just realized the form only has date completed and my phone number. There is no where to sign. Is that a mistake?

Hi Avila89,
It's been a while since I had to deal with these Social Security disability reviews but with MS since I was in my late 30s, I have had to do quite a few of them. I can think of one reason why you should definitely include any hospitalizations/surgeries on the form: those are stressful situations, showing how much stress you've been having to put up with. It could help your case, seems to me.

Just my two cents here.

who knows why they want some things and not others. most people who work there dont even know. if there is no where to sign dont sign it.

i havent filled one out since 2015. only got two short forms in 20 years. none for last 8 years.

The New york state guide has helped many people. unfortunately is hasnt been online for a couple of years now.
