received short form; reason for visit inquiry?
 
Hello,

I was diagnosed bipolar & panic attacks, but I filled out my last 3 reason for visits as such:

3/15 Depression
4/15 Anxiety Panic
5/15 Severe Depresion (there was no space for the 2nd s.)

Since then I've read to write the original diagnosis which is what I basically did but not the actually wording as my bipolar condition swings from manic to depressed which I am currently. I haven't sent it out yet; should I just wait for the 2nd form to arrive and change it to:

Bipolar
Panic Attacks
Bipolar

I've read that if at least one of your original diagnosis aren't listed it get kicked out to someone. Will the word Panic suffice. I also had to fiddle with month 5 as I originally wrote it as 6 but it was kinda easy to change but might not read on scanner.

Received Social Security disability CDR (short-form) yesterday
 
I received my social security disability CDR (short-form) yesterday. I think I filled everything out correctly except for question #5b. I did not enter a diagnosis but put instead.

1. Counseling 05/15
2. Counseling 05/15
3. Prescriptions 04/15

Any feedback on how I answered question #5b will be greatly appreciated.
