Please help Bryanna……… infection after Wisdom teeth extraction
 

Under desperation, I searched internet and found this web site and see what a wonderful person Bryanna is to this site.

In July 2009, I had my four wisdom teeth extracted. After the surgery, the other three were recovery well except for the upper left side one. I had mild pain on tooth #15 after the extraction which I thought it was normal since it is near the extracted site and the needed time to be healed. But after more than two months, the pain was still there.

I called the oral surgeon office to ask if it was normal to have the pain. I was told to come to his office. On that day, the X-Ray shows everything was normal, the tooth had no fracture, the gum healed well, but as he tapped the #15 tooth, it hurt. The surgeon was not sure what the problem was and he sent me back to my family dentist.

The next week I went to see my family dentist, and was told everything is healing well and just needs more time to settle. But the following week, there was a bump on the gum right on the top between the extract site and my #15 tooth. So I went back to my family dentist again and I was prescribed 500mg Amoxicillin.

He said maybe there had some minor infection in the gum. Since then I had six courses of different antibiotics to take care the infection. When I took antibiotics, the pain on the tooth and the bump will go away. But around a month to six weeks after I finished the antibiotics, the bump and the pain will come back. I went back to the dentist and got another course of antibiotics. During all the time, the dentist took a couple of X-rays and did not see any abnormality on the tooth.

The gum on the extracted site also seems had healed well. In December, he referred me to an Endodotist to see what the real problem is. The Endodotist did some test and concluded that root canal need to be done to resolve the infection and the bump on the top of tooth # 15. After the root canal, my #15 had more pain than before.

I called back to office and was told this is normal post surgery effect and was prescribed antibiotics for 10 days. 9 days after the root canal, I still had bad pain. I went back to see the Endodotist. He told me that my #15 had very bad infection and he can see couple of sinus traction.

He suspected that my #15 had a fracture. He sent me to an oral surgeon to take a look what the problem is. During the examination the oral surgeon press the tooth from the side and it was hurt. And then He asked me to blow my nose as he closed and opened my nostrils. The oral Surgeon concluded that I had two issues going on in the same time. My #15 is fractured.

And I had a hole between my sinus and the wisdom extraction site. According to him, there had a bubble come out on the gum where the wisdom tooth extracted when I blow my nose as he close the nostril. He told me that there is a gap created between my sinus and mouth because my bone did not grow and close properly after the wisdom tooth extracted. The reason the bone cannot close the gap because there is a skin or tissue fall into between bone and blocking the bone as it tries to close the gap created during the extraction.

That is why the antibiotics didn’t take care of the infection and the bump. So he would pull the #15 tooth, and would cut the tissue that blocking the bone, and fill the gap with fat that he cut from my cheek. What he said seems make sense to me, even though all the other doctors didn’t mention anything about the hole. We decide to do the surgery.

Before and after the surgery, I was priscribed antibiotics. After the surgery, the doctor told me, the #15 was not fracture, but it had been badly infected. It had cyst. Now six weeks after the surgery. I had pain on tooth # 14 which was a perfect tooth, but also had pain around the ear, and down on the jaw bone. But every time as I went back to the oral surgeon, he said it’s normal pain after surgery.

He closed my nose and asked me to blow my nose, there had no bubble came out. He asked me if I had any sour taste or drainage coming from my mouth. I didn’t have and never have even before the surgery. He sent me back to my family dentist to see if I need alignment. After meeting with my family dentist, he said everything looks OK, the gum heal well, no sign of infection.

But I know something is definitely not right. I had the pain on #14 right after the surgery. Some time, I feel sharp needle pain coming from #14 gum area. At beginning, the pain is not that bad, when I tap or push down the tooth, I did not feel the obvious pain. But if I use the bottom teeth to push the #14, I can feel the pain. Now six weeks after the surgery, the pain on #14 is increase. The pain increases much more starting after the fifth week of the surgery. The pain is just on the gum area facing my check.

The gum looks not red or infected, but if you push the gum, it hurt. Even with a gentle bite, I can feel the hurt on # 14. The pain is a sharp pain. I expect I will lost the #14 very soon.On my face (around jaw join) close to my ear area, I also constantly feel the burning pain. I also sometime feel the pain under the left ear. Some time the pain is going down from left ear to the left neck muscles.

All these pain I didn’t have before the surgery. It seems the pain is from my nerve. I know the pain is related to the nerve, but I don’t know how surgery on the upper tooth can cause all of these areas hurt. Now I really don’t know what the next action I should take. Who should I see?

Another oral surgeon or prodonist? I am worry if this is a very serious problem and will continue getting worse. I had a healthy teeth and never had any teeth problem before the wisdom teeth extraction, even any teeth filling. I don’t know if there is any end of these nightmares. If anyone can give me any advice, I truly appreciated it.

Just saw another Oral Surgeon. He thinks I have ATN. I did't have the sharp pain before the last surgery. can you please share your experience if anyone has ATN.

Hi Shinta,

Many things could be happening at once. At this point you may have some form of neuralgia developing. It could have started out with the wisdom tooth extraction but that would be difficult to determine since you had a problem with tooth #15 as well. It could also be a virus called VZV which is Varicella Zoster Virus which is a herpes simplex virus. This virus has some visible variations and it is too complicated to explain properly on this thread. I would either see another oral surgeon and/or an Ear Nose and Throat specialist who is familiar with facial nerve pain.

I feel terrible that you have gone through so much with your teeth! I would not wait to have this re-evaluated and certainly before removing tooth #14.

Hi Shinta,

I didn't see this post when I replied to your other one. Ok, so you saw a different oral surgeon. Then I would recommend seeing an Ear Nose and Throat specialist and tell him the entire dental history beginning with the removal of the wisdom tooth. Make sure to mention the sinus perforation!!

Too all those who suffer from nerve pain in this case dental.i a have been through the gauntlet in dental pain.i have rsd/crps full body for 12 years.i was electrocuted.my surgeon who performed 15 surgeries on me, him and another dr.stated all my nerves are in flamed.i have been in chronic pain for nearly 12 years.all the usual symptoms and syndromes of rsd.but some of the sensitive and severe pain is you guessed it my teeth.right now if i drink a cold glass of water my whole lt. Side of my mouth flairs up.it is a pain you can do nothing about.i have been too the dentist and specialists.i kept coming out worst than i went in so i stopped going.after they pulled one third of my teeth and charged me after insurance did not cover.i realized i was the dummy.anytime they have too cut or stitch its surgery.and bam their goes the price.ins.wont cover that.after breaking my wrist and having 5 pins and 8 tittanium screws placed in it and then electrocuted in the same wrist with the hardware still in i thought i had felt the worst of all pain.nothing topped going and getting a tooth extracted and having too put a stitch in it.the pain went too a ten after the novcaine wore off.percocet 7.5 did`nt touch it.i finally had too get a stronger med before it settled down.but i can truly sympathize with you guys and gals on nerve pain and your teeth ive been there,it is horrorible too say the least. When I went to see another oral surgeon, he gave me a shot beside the ear to numb the joint. After the shot, the first three days, my pain reduces a lot, then increase after that, but still better than before. At the same time, I also started to take Vitamin. The Surgeon told me I have ATN, but more on the joint. So he sent me to see Dr who specialized in managing Facial pain and TMJ pain instead of Neurologist.
Since last week, the pain on my left face right below my cheekbone increase a lot. IS this related to the nerve issue or the sinus perforation.
Now the new doctor put me on temporarily splint. Today is the second day to wear it, I start to feel my bottom left molar teeth hurt after I wear the splint. But if I took off the splint for a while, the pain went off. Is it normal for me to have the pain? Do there have any side effect to wear splint for a long period of time? I read in some place, sometimes wearing splint can cause joint problem. I just don’t want to get any more problems. Any comments will be appreciated.

I am so sorry you are you gking through pain especially in your mouth I know that has got to be the worst that would be the last place I would want RSD or CRPS. So I just want to make sure I read this right you had know symptoms of this dreadfull disease until you had your wisdom teeth removed? As far as I know only very rare cases and I mean like 2% start at a body part that isn't a limb. I had my wisdom teeth removed about 3 months after I was diagnosed at the time I was only stage three and since my teeth were impacted so bad if they got any worse or if the surgeon ****** up at all my mouth would feel the same way my leg does. Thankfully it doesn't feel that bad **. What I'm trying to get at is that my jaw and gums still bother me a lot nothing like they used to but enough to make me very uncomfortable is what bothers you the teeth and gums next to were you wisdom teeth were or is it the gums were they were? I had mine out about 4 or 5 months ago and I am also a smoker so it will take twice as long for everything to heal in my mouth. How old are you and How long ago were they taken out? If it is RSD and I were you I would go to your dentist first and asks for pain pills only something with tylenol or ibuprofen will work for mouth pain and then go see a neurologist at a pain management practice for a more permanent solution if your by or in MD's or surrounding states email me and I can tell you best doctors cause I've seen them all I know what really bad pain is I've had it in my mouth too before I got them removed if you've ever had braces or if your teet5h grew crooked from your wisdom teeth being to big for your jaw it honestly might be what I aam feeling and that is your teeth moving back into the place that they used to be in. What type of pain dp you feel explain it all iin detail and I can tell you based on my own experience. Hope you feel better. Pain meds like percocet 10mg with 325 of aspirin.
