Dear Mrs. D:
I am a patient who suffers with Small Fiber Neuropathy which is at its worst in my hands. I also notice that quite often my hands get fairly cold (almost like Reynards but without the associated color changes in the hands). That said, I also notice that when I am on Cymbalta 60mg daily that the temperature of my hands seems to normalize. Would you have any idea why Cymbalta (in addition to relieving the burning sensation in my hands) also seems to literally warm them at the same time? Thank you! David |
Cymbalta affects two neurotransmitter systems. Norepi reuptake and serotonin reuptake.
Serotonin, is a vasodilator, and most of it is in the body. It is a biogenic amine with many actions. So Cymbalta affects the body as well as the mind. Serotonin is complex and does many other things as well: http://en.wikipedia.org/wiki/Serotonin But common side effects with drugs like Cymbalta...reflect the increase of serotonin in the gut (nausea/diarrhea) and skin, vasodilation. People with Carcinoid ( with high serotonin levels) have very severe flushing, as one symptom. This comes with a feeling of heat. The norepi part, of the Cymbalta, raises blood pressure, and stimulates the sympathetic system to some extent. The effects on the bladder..causing urinary retention ...is part of this action. Depending on your particular genetic make-up regarding neurotransmitter balance, you will have certain side effects and not others. Some people are "high serotonin" types, and therefore their side effects would be different than the lower types. High serotonin types often discontinue SSRIs and SSNIs (like Cymbalta) due to severe side effects. |

For the record, I found Cymbalta to be very effective for my small-fiber neuropathic pain on 60mg / day. Indeed, I got about 75% overall relief of my burning and tingling symptoms! I also had virtually no side effects. I have now been off Cymbalta for 15 months due to improved symptoms and have been medication free (yes, people with small-fiber neuropathy can improve!) |
