Calcium with Vitamin D3 Deficiency
 
Dear All

Myself Aged 39 years From India , went to hospital for the Treatment of My mother,

Incidentally There was Free Camp for Bone Marrow Density Test ,

Since It was a Free one , I went for a Test.

For my surprise , I got the Bone Densitometry Report Says


My Readings

Speed of Sound: 1480


T-Score : My Reading -2.90 (minus 2.9) -------> T score Below - 2.5 which indicates the osteoporosis.

This indicates that , am suffering From osteoporosis

Seeing the above Reading , Then Doctor (orthopedic specialist) Advised me the Following Tests.

Vitamin D Total (25 oh) (serum) : My Reading is 22 ng/ml ,(Which Falls in the Midly Deficient Range)

Reference Ranges Are

Mildly Deficient : Range 15-31.9ng/ml


Optimal : 32- 100 ng/ml


Consider Toxicity > 100 ng/ml


Also

Sr Calcium -----> My Reading : 10 mg/dl (is ok) Ref Range : 9 -11 mg/dl

My Questions:

Now I started Experiencing Pain in the Back , Top Back , mid Back and Low Back ,(Not Severe)

Is this is due to the osteoporosis that is deficiency in Vitamin D ?

How Long it takes to cover the contents,

How serious is these Findings

Should I need to Consult the Neuro Specialist ?

Any thing Wrong with my Neuros

How can I know, whether My Nerves System is Normal or Not ,

How To measure the strength of My Nerves ,

1. Uprise -D3 60k -Vitamin D3 Capsules 60000 IU ----------Weekly One Tablet


2.Instrel Tabs -- Tramadol hydrochloride & Paracetamol ,By Dr Reddys Labs

3.Sonipraz D 40 --- 30mgs of Domperidone and 40mgs of Esomaprazole

4.Ezorb Forte Tablets ------- calcium aspartate 500 mg, calcium orotate 500 mg, calcitriol 0.25 mcg, zinc sulphate monohydrate 7.5

mg, magnesium hydroxide 50 mg, hydroxocobalamin 100 mcg, folic acid 1.5 mg.
5.Neuromin - OD -------Mecobalamin 1500mcg

In the US with a younger patient, your diagnosis would probably be osteopenia. It is basically low bone density not due to aging.

In your list of drugs ....is that Sonipraz D 40 --- 30mgs of Domperidone and 40mgs of Esomaprazole.
Have you been taking this for a while? Acid blocking drugs reduce absorption of calcium from the GI tract. Therefore they are triggers for bone loss and low density.

Also in your drug list is this:
.Neuromin - OD -------Mecobalamin 1500mcg

This form of B12 needs to be taken on an empty stomach to be absorbed properly. So make sure you do that.

B12 is a cofactor in bone metabolism and needed if you are low in it. I don't see that you were tested for a B12 level?

In the US some doctors here are not recommending high dose calcium supplements with Vit D supplementation. The dose recommended is 600mg/day or less. But things do vary from country to country. Once you normalize the Vit D levels, you should be absorbing calcium from food. If you don't eat foods that supply calcium then a supplement is needed.

I can't really address your back pain. This can be due in older patients to the collapse of the vertebrae and hence there can be compressions of nerves that leave the spine and cause pain when compressed. That can only be evaluated with spinal Xrays and/or MRIs.

Back pain may be due to muscle strain, poor posture, gall bladder disease, or osteoporosis of long duration. Even some cardiac symptoms may refer to the back in some patients. Therefore, I think more evaluation by your doctor is needed.

All Mentioned Drugs , I have started Taking since Monday 27.08.2012

But there is difference ,that is

Sonipraz D 40 ---My physician has advised me to take empty stomach That is Before Food ,which am following


Neuromin - OD --- Which am taking after the food , as per Physician advise

Your doctor does not know that B12 is not absorbed well with food. If you want it to work for you, take it on an empty stomach. The B12 will also not be absorbed well with the Esomaprazole since blocking acid blocks intrinsic factor. Then the B12 has to be absorbed PASSIVELY, and presence of food blocks that by absorbing the micrograms like sponge. Empty stomach is best. If you choose not to follow this suggestion, you risk not getting benefit from the B12.

The Esomaprazole blocks absorption of B12, folate, calcium, zinc, iron and magnesium. All these nutrients are dependant, on stomach acid.

Most doctors haven't a clue about the consequences to the body that many drugs can cause. This information has been published over the years on PubMed from researchers around the world. Now it is available on the net commonly. (I have it in book form and use it as a reference).

Why do you need the Sonipraz D 40 at all? It will interfere with your calcium supplement. It is now known that proton pump inhibitors like this one cause increased risk of bone loss:
