Hsp and peripheral neuropathy

I have been battling presumed HSP ( hereditary spastic paraplegia) for 25 years and with exercise a baclofen pump and pt have kept my ability to walk and work but over the past 6 months have developed severely painful peripheral neuropathy that is minimally treated by neurontin. Does anyone have knee pain with their peripheral neuropathy cause it's pretty severe and unexpected. Recently started Effexor as well which is less tolerable but am looking for anything that will help. Any similar experience or advice is appreciated. Thanks sk
