Hi Everyone.Back Problems 2nd app for SSDI Need help on the reports Please
 
Hello All,I have had back problems since 93 I am 49 going on 50 Sept.1 I have not been able to work since 2008 was denied SSDI the first go around from 2010 and just applied for the 2nd time.I know have a written statement from a spine surgeon that said he will help me out with what ever we need because he says I AM Disabled.I just received the Pain/Fatigue,Adult Function and Work History report 3 days after going to the SSA office and applying and need all the help you all can offer to help me fill these forms out and say what they need to hear.Its been so tough on my Family and I really,really need to be approved this time because the SSA girl said I would be out of the window for SSDI SSI soon.I pray everyday.Please reach out and help me.Its so important.On the function report the questions what do I do from when I get up till I go to bed & How do your conditions limit your ability to work.With all the meds I take I just cant think and I want it right.I have 10 days to get it done.God Bless,Born

Hello All,My medications dont let me think the clearest.I really need some help with the Adult Function report Questions:How do your conditions affect your ability to work and Describe what you do from when you wake up to when you go to bed.This is my second app for SSDI .Help if you can please

A friend or family member should be able to help you write & describe what is needed as they would know your limits better than us.

But the sticky thread should have some good tips and help for you.

I hope the members can suggest helpful things also.

Born,

Many of us understand the desperation behind your words "tell me what they need to hear...." We CAN'T give you THE answer, because there isn't only one correct answer to any of the questions.......and the possibility of not accurately describing YOUR condition could be fraudulent.

Because it is so difficult to thoroughly and concisely as possible get all of the facts across, you might want to type out your answers so that you can cut and paste, or add new facts when you remember. If you do that and post parts here, we might be able to offer helpful suggestions to elaborate on some points, or parts that need to be more clear, etc without putting words into your mouth.

You can also have a family member or friend help you complete the form as long as you document who helped and why you needed assistance.

Hello Borninabarn,

Just know your not alone. I just submitted my completed forms to my attorney last week, because they wanted to go over them before they went to SS. Personally it took me over 3 weeks to complete them because of my disabilities, but if you take it slow you will get it done.

If you have the equipment at home you can scan the forms into your computer and type in the answers (that's what I did) that way you can go over it slowly and make changes if you need to.

If doing it on the computer is not an option and you don't have anyone who can help you call SS they WILL give you extra time and/or they WILL assist you with the forms. Also if you have an attorney they can help you with the forms.

Right now you're ahead of the game, because there are people on this forum that can at least give you support and understanding that our friends and family will never completely understand. I wish you all the best as you go through this process. :grouphug:

I am not sure this will be of any help as I have not yet been approved and am awaiting my hearing before ALJ but I can tell you how I handled the form. I wrote the time and what I did from getting up and going to bed for one day and submitted it as an addendum. Example: 7 Am Up and put coffee on. 7:30 drank cup of coffee 8AM too tired to start day so watched TV for an hour. 9 AM Worked on this form 10AM tired took nap Noon Up again and making lunch for myself -sandwich. Etc. Etc. until I went to bed.
Maybe some others here can chime in and give an opinion on whether this was a proper thing for me to do. Like I said, I have not yet been approved. If nothing else, it will give you a guideline to assist you in completing the form. Many evenings I get in bed wondering what happened to the day. I am totally exhausted and can't remember getting anything done. I figured they asked what my day was like so I kept a log of one typical day and gave it to them with the form. May have been a WRONG decision but that is what I did.
