question about the dates of disability decisions?
 
When you get the CDR long form (SSA-454-BK) it has several questions about dates that refer to ...........................
............................ "the date of your last medical disability decision".....

The questions are mostly asking about work you may have done, education, treatment..etc.. you have had since that date.

My question is:

1. Is the date they are talking about the date that I originally went on ssdi (was over 20 years ago).

OR

2. The date of my last CDR (only 5 years ago)?

I really need advice from someone asap........thanks.

Hmmm....that's a good question. I'm assuming that "the date of your last medical disability decision" would be your approval date, but I don't KNOW that for a fact.

Some of the posters here are very knowledgeable about the SSA's paperwork. I hope one of them will see your post soon and be able to answer for you so we'll all know for sure.
