Signature spot on disability function report?
 

I got a mailing from social security - a big function report to fill out. I had just got one from the Standard Insurance, with similar questions. (They are the disability insurance co. through my work place).

I filled this thing out, I hope I did a reasonable job. My SSDI attorney specifically advised me not to add additional pages with detail - her point being if I was able to sit and devote many hours to doing a very thorough and detailed report, it might appear I was able to focus and could hold a clerical position with no problem. I hope it's not bad advice. She has stellar ratings on Avvo. :confused:

Anyway I see on page 8 there is a spot to fill in your name, address, etc. I see no signature line though. Am I missing something? I thought with something of this gravity, there really ought to be a place to sign & affirm your statement.

I know I can call the social security number on monday or my lawyers office - just thought I would ask my friends here first.

It has been almost a year since I completed my SSDI function report but I am pretty sure it had a place that you had to sign it.

Disability lawyers are usually pretty familiar with the office and judges in their area so I would never second guess him/her.

I have read many articles and most of them say be detailed and explicit. I added a lot of pages with details. (Maybe that is why I was denited.)

I have come across a limited number of articles supporting the idea of limiting your responses.

Which is the "correct" advice? I guess it really depends on the office with which your claim is handled, maybe???

Hopefully others with much more knowledge and experience will chime in. I am clueless and just going by some of the material I have read.

I did my function report long before I secured an attorney. I actually did not get an attorney until AFTER I filed for a hearing and then knew I would need assistance.

Look at the bottom of the FIRST page. If my memory is working, I think it might be there.

It took me weeks to complete mine with all the added pages. If they think I can hold a clerical position just because I could "add" detailed pages, they do not know how LONG it took me to do that.

They know how long it took me to complete the application because I did it online and it took weeks. I guess they can track how often I signed back in to work on it.

Raindrop, in my case I am feeling my long drawn out response to the "describe your typical day" may have been the difference that got my award. I spent 10 days mulling it over in an attempt to convey what my average day (24 hour period) is really like, wound up taking up 8 pages. I began my process in late May with ZERO medical records relying solely on the two doctors SS sent me to see, and now, less than 4 full months later I received my award letter yesterday!
