Muscle Cramps, Dystonia, Stenosis: Help!

I posted the following in NPF's Ask the Doctor Forum, but wonder if some of you might have had something similar and could share your experiences:

I hope you can help me with a dire situation that is causing me much suffering and distress. PD Dx 7 years ago, tremor dominant. Last few months, low back pain & sciatica symptoms on R. side, with severe hip/thigh pain when standing plus intense muscle cramps (dystonia?), some so severe I scream in pain, esp. at night. Recent MRI showed focal (canal, central) lumbar stenosis. Epidural gave little & brief relief and pain pills caused constipation, so spine surgeon consult now scheduled. I'm unable to stand or walk without walker & only for a few steps.

Meantime, for some relief my primary doc Rx Neurontin (I haven't taken it yet), and neurologist Rx Baclofen and said to increase Sinemet. However, it seems the Sinemet actually causes muscle cramping, though it helps with energy & sleep. Might it help to take smaller doses at more frequent intervals? Or add Amantadine? Also, is it okay to take both Baclofen and Neurontin, as well as Motrin or Aleve? I'm concerned about possible interactions or side effects. I'm being encouraged to get DBS; would it help with dystonia? Is it possible that I might avoid spine surgery if DBS and/or meds were optimized? Thank you.
