I would think normal mail is ok - just keep a copy - i looked at the form and it fairly basic questions that they should be asking - If you have not rtw or gotten better then what is the harm. This is nothing compared to WC and LTD intakes!

certified mail.I am hoping that it go's though the scan process with out getting kicked out and life can continue as it was.Crazy how one little 6 question form
can cause so much stress and anxiety. Now it's hurry up and wait time.

Agreed, same form same stress..but then again many of us here stress over simple things in life so something this important just doesn't seem like a "Little" thing, its our life! I can get stressed out when I get a phone number on my caller id that I don't recognize to the point, I am freaking out! I dare say many on here are like this as well.

HOpe the machine scans it and you get the Positive notification!

Trust me, reading these sites, you aren't alone!

hello all
well as I posted before i mailed out the SSA- 455 form.

OK so they want to see the latest medical record so be it.this whole thing sorry to say is really pissing me off.I fought for years and $5000.for legal representation to get my SSID. A Federal administrative law judge determine I am eligible.my condition has worsen since then and will continue to.So why do i have to go though this crap all over again.
If I really could go back to work I would.went from make 60k a year before my injury to **** sorry of the language just really upset over this.

The SSA is obligated to try to confirm that we are still disabled every few years.

For those of us who have been disabled and are now financially dependent on our monthly SSDI/SSI checks, those minimal inquiries can stress that feels like an attack on our health or integrity.

When this round is over you can relax for a few years until the next review period.

Hello all
It has been awhile since I wrote from my last post. It's Been that long waiting to hear any thing.
Update on my medical review.
I received another form V 13 from the state health department. Is this normal?
more or less the same form I filled out from the last one.One thing I did do was file for (LIS) Low income subsidy so that I could change my medicare insurance A-B to a different insurance plan before open enrollment of October.I wish I did that a long time ago I am saving a lot of money as far as medical expenses go's.
My whole out look on that was being on a fixed income I couldn't afford any more costs for more insurance but come to find out I don't have any cost for the new insurance.
I have been waiting to get gallbladder surgery for months and couldn't afford the 20% deductible but with this new insurance it's going to cost $75.00 and will happen here some time in September.
