SSDI filing sample forms

Hi all! Does anyone know where on-line I can find current sample forms of the on-line original SSDI application, as well as the original disability report sample form (two items required for initial SSDI filing)? On the web I found a downloadable sample application but it is from 2006 and I know it is old... because I started the application on-line and it is different. The downloadable sample disability report I found is from 2010 so possibly that is current...? thx!

JUST A THOUGHT.......

You may be able to pick up some at your local SS office. If you have a home scanner/copier, you can make many copies to practice on.

tx Mz - I may resort to that ... I was hoping to find one on-line as I think I would have to wait in line at SS.
