Question about Function Report- Adult
 
We live in Wisconsin and my son suffers from Autism. Per his psychologist and DVR, we applied for SSD for him. He can not get a full time job as he doesn't understand the questions at the job interviews. No one will hire him. He has a part time job of maybe 4 hours a day loading truck.

We got the Function Report for him to fill out. There is no way he can fill this out as they would never be able to read his writing. He writes at about a 2nd grade level. My question is should I fill it out for him alongside him or should he fill it out in his own handwriting? I don't know what would be better. He would fit maybe 3 words in those lines with the way he writes.

Any advice would be appreciated. Thank you!

I should have added that he is 20 years old.

You should contact the analyst that sent him the forms and ask.

I think it makes sense to do both; make two copies of the form - one he answers in his own handwriting and one you write down his answers.

I think there's a place on the form, if I remember correctly, or it was in the instructions that you can have someone else fill out the form for/with you. I think you then sign as the person filling out the form...and there was a place for that.

Hang in there. It's a ridiculous process with way too much paperwork in my opinion. I was lucky enough to get it on the first try but I feel for those people who go through these mountains of paperwork only to be denied. It's not easy but don't give up...I think in many cases they want you to!

Get a disability lawyer, if you dont have one.
