Recieved my first short form 455
I not sure how to fill the question 5
Reason for doctor vist i was approved on neurocognitve
Disorders and depressive, bipolar and related disorders
There is 16 boxes per visit i cant fit
Neurocognitve disorder in boxes or do they want simple reason such as head aches, and my reasons for visits i gave traumatic brain injury and have many cognitive problems just not sure want they want to see in reason for visits
Or what diagnosis is for going to dr
I dont want to fill out wrong and raise red flag
I have been to dr seven times with the the three year review period not worked at all and condition is the same abd take all prescription

put down in reason for visit the name of the illness or illnesses that you were approved for ssdi. so for instance neurocognitive disorder is what you would write for the visit. if the name doesnt fit in the provided boxes stop writing when you run out of boxes so for example neurocognitive d and then stop. dont write outside of the boxes. dont write in the comments section. if you have seen the doctor for the other illness or illnesses you were approved for also list one of those, whatever is your case. just make sure to fill out three visits total. and again the reason is the name of the illness you were approved for.

I just got the short form for the 2nd time in two years. Just when my disability is wreaking havoc on me I get this in the mail. :mad:

We understand but they are just doing their jobs..

I suffered a TBI and brain damage, yet I still get questioned over 12 years....Anyone know of a way to reverse brain damage Im all ears! lol

Filling out form 455
 
I filled out the short form today and when I got to the end I entered my home phone number. Then it occurred to me to look at the phone number they had listed on the front of the form and saw they had my cell number. Will that cause the computer to kick out my form? Is it okay to white out the number I wrote and change it to the one they had? Will white out on the form cause the computer to kick it out?

dont white out anything....that will most likely get it kicked out by the computer. a change in phone number shouldnt be a problem. many people change their number over the years.

Just got my latest short form...(Been probably 3 years?)..From my experience I expect the long form next, then the Dr. after that.

It gets so old they think brain damage will get better....sigh
