Accuracy of patient reported symptoms and history critical .. correct?

I have recently had a recommendation for surgery, which I'll get to later. (I am very against that until much more evidence). However , I have been searching the forums as much as I can and I want some feedback to see if I understand this correctly. It is the correlation between the patients (self reported) symptoms+ history AND the diagnostic findings from MRI, X ray, exams that should be the determining factors on decision. Y/N. Now, if that is correct; I see that my symptoms, history, etc. seem to vary quite a bit in each medical record as they travel from DR. to Dr, consult to consult..... That seems like a big issue to me, am I correct? Also, I had a good symptom journal on my phone, which of I lost; now I have to reconstruct my symptoms history from memory (with help from some logs.)
So before I ever ask the forum to comment on MRI aspects, I guess it is critical that I ensure my history+ symptoms are as accurate as possible first before I worry about anything (else like MRI)??? Y/N
. I'd like to get feedback on the above. Anyone feel free to take a shot, and maybe If I'm lucky, someone might want all the background and info for mri open mic! Thx:)
