Question #6 CDR
 

Hi all,

First post here. I was wondering if anyone knows if a partial hospitalization program(voluntary outpatient psychiatric program) would count as a yes to question number 6 on a CDR?

Also does the SSA request all medical records for a long form review or do they only have what you give them? Thank you.

First question you asked on short form cdr, if the partial hospitalizations is for an illness or condition, or related to it, that you were approved ssdi /ssi for then yes.

second question for a long form cdr, they request medical records directly from the providers that you put on the long form.
