Similar drug names increase med mix-ups
Researchers have found that errors from sound-alike, look-alike drug names are a problem that spans the entire health care industry. Medication mix-ups have more than doubled since 2004 and have caused harm and even death to some patients. In one example from the study, an 8-year-old girl died after receiving methadone, a narcotic, instead of methylphenidate, an attention-deficit/hyperactivity disorder medication.
