i asked my chiro about this and she said that prior to full chiropractics they used to only address that top joint but now they address all the joints. she said that when i see her she manipulates that top one plus many of the others, especially the neck and thoracic spine. she said these are important to opening up the ribs. so i didn't proceed further in looking for someone that specializes in this. but it would be interesting if one of us tried it out. it was so nice to hear that for Montel and others a few sessions completely changed their lives around.
