Hello,
You should be able to find one thru your Insurance website or booklet.

Were you face up or face down when this happened?
"she held me up by my arms off the floor"

I would think an expert physical therapist would be a good idea, unless your main doc really thinks a physiatrist is needed.

[Physiatrists, or rehabilitation physicians,
are medical doctors who are:
* Experts at diagnosing and treating pain
* Restore maximum function lost through injury, illness or disabling conditions
* Treat the whole person, not just the problem area
* Lead a team of medical professionals
* Provide non-surgical treatments
* Explain your medical problems and treatment plan
* Work not only on treatment but also prevention]
that site has listings- here's FL

Welcome from me as well Krandog.

Unfortunately I'm unable to suggest anything further to what Jo55 has already suggested as the information that you've provided may have been a little short, but please check out the link that was provided.

I really do hope you get the answers that you're looking for.
