Default ketamine infusion procedure codes

Hi,
I may want to proceed with 5 days out patient ketamine infusion with Dr. Thomas Leverone if my pain is not under controlled after a series of blocks. My pain has gotten out of hand after a recent blood drawn and an unexpected hard slam into my back by a door. I was so stupid to stand by the door texting my daughter. Can someone provide me with the procedure codes? I want to check with my insurance to see whether or not they may pay some of the cost?
When i called my insurance today and asked them whether they cover ketamine infusion, they said "Yes". However, they said i should try to obtain the procedure codes and check back with them. Sound too good to be true!
Thanks for your help!

[QUOTE=numb;812735]Hi,
All of the codes along with all info related to ketamine is available at The Ketamine Klub for RSD/CRPS patients on FB. They have compiled all info in one space including protocols, doctors, codes, articles, etc.

I will check it out and thanks for the info. I did call the facility and they gave me the codes. I then check with my insurance and they said it will be covered 50% because the provider as well as the facility are out of networks.
