Default function report

i sent in my report in their envelope last week and today i get a second notice saying thet didnt get they were closed b the time i got it so guess i will call in morning they also sent a new letter saying what the reason medically i am applying for and they listed places they are sedning to get info , and said i amy have to come in for meeting or exam. i hope wheni call they say they got it cause i would hate to fill out again,

and alos should i take my drs o rfc form myself to fill out too? or will ssd send them one?

It's always best to make copies of everything that you send. With the function repport it's best to write as much detail as you can. Use attachments when there is insufficient space on the form. It's always better if you get your medical evidence and submit it to the SSA. The claims examiners are way too busy to follow up with doctors that don't respond.
Get you doctor to fill out an RFC form (Residual Functional Capacity) that describes the limitations that prevent you from working. **

Also wdwgranny when sending something send it either with a delivery confirmation or a signature confirmation. That way you have a receipt what date they got it. Keep everything you get from SS in a briefcase or some kind of folder. This way if they claim they didn't get something you have documented proof.
