I was suprised i just received another form called "adult function report form" and a form for my mom to also fill out. Any tips on answering the questions ? Any tips for my mom to answer her form?

I checked out the form online & the questions asked appear to be pretty basic.
Answer the questions to the best of your abilities and be honest. The form comes w/a section on what to do if you need help.

As for the form your mother received, you never said what the form was. Name of form?
Have you checked the SSDI website?
