CDR Short form

I am stressing over this CDR review process. I recieved the short form 6 question form in the mail recently. I filled it out and mailed it and realized after I mailed it that I should have put on the form somewhere that I was going to see my doctor for my disability next week. Now I am worried that it is going to be flagged when they scan my form. My profile score is really low and I have been blind since birth. What are the chances that my form is going to be kicked out? The is the first time that I filled out the short CDR form so I don't know what to expect. I can't wait until this CDR process is over. Any replies on this would be appreciated. Thanks for answering my question for me.

pawl this is from SSA about how they process the short forms. question 5 is the question about doctors visits and ths is what it says. it looks like from reading this that they may contact you by mail or phone because you have not indicated any visits but indicated no medical improvement. hope this helps.

Date of onset

Date of initial decision

Date of latest decision

Reg basis code

Vocational rule met

Diaries set

Blindness

Assess the importance of ongoing medical treatment, or its absence, to the status of the condition. Read carefully the “how often” and “reason for visit” narratives and draw any conclusions.

Consider any remarks or attachments that might address whether medical improvement is or is not indicated.

Consider whether a telephone call to the beneficiary or representative payee will provide further details.

For LOWS, recommend a DO CDR only if the narrative/telephone call response(s) indicates there is possible medical improvement.

For HIGHS/MEDIUMS, take a DO CDR action unless the narrative/telephone call response(s) indicates no likelihood of medical improvement.

All Profiles-If not explained on the form, obtain by DIRCON to the beneficiary or representative payee the reason(s) for the lack of ongoing treatment, especially if impairment codes are:

4140 (angina)

4430(arterial)

5710 (liver)

5840 (renal)

2500 (diabetes)

These impairments usually require continuous medical intervention.

Recommend a full medical CDR only if the DIRCON response indicates there is possible medical improvement.

Disability Update Report
 
Hi I Just Got This Disability Update Report Form, SSA-455, That Has 6 Questions On It. I Filled It Out. I've Been On SSI For 13 Years Now. I Received One Of These Before Back In 2005, Now I'm Getting Another One. I Have A Question, If Anyone Can Answer This For Me. In The Remarks Section, They Only Give You A Few Lines To Answer Any Questions Further. Which I Did, But I Ran Out Of Room, So I Added A Few More Piece Of Paper To Further My Questions In 5a & 5b, Plus I Also Added My Doctors Address & Phone Numbers, I List All The Meds I'm On. I Won My Case For A Mental Disorder. Do You Think That Giving Them More Info Will Hurt My Case Or Better My Case. Also I Made A Mistake On The 4th Question, Where It Says, Which Describes Your Health Now Compared To August 2010. I Put A X In Worse, But I Put White Out On That X, & Put An X In The Same Box. Do You Think Because I Did These Few Things I'm Going To Get Flagged. Can Some One Help Me With These Questions. I Didn't Do Anything Wrong I Just Corrected The Mistake I Made.

from what i have read you may well get flagged.

here is a guide from New York State on how to fill out the 6 question cdr.

you didnt need to put in the doctors names and addresses and the list of prescriptions for the short form. you really should not fill anything in the remarks space never mind adding additional pieces of paper.

Giving honest answers to questions is never wrong, it is right.

Since you attached pages to the form, a computer cannot read what you wrote, so it will have to be read by an employee. You may get recontacted for more information. It is possible that SSA will look for your current medical records and do a full review. Not in your control.
