Default Question about 6 question cdr form

Hello everybody, its been awhile since i have been around. I was on the old site but for some reason i had to re-register here? How did you all keep your handles from old site?


Okay i got a couple of questions and i know that somebody here will provide me with good answers, because i have always gotton good info from the board.

I just got my 6 question cdr in the mail today. My last cdr was the long one.
First my disability was based on neck surgery that i had, i have a cage in my neck. I also had some mental problems but for the most part its based on the neck.
In looking over the questions a few of them seem tricky or am just making them out to be tricky.
Question 5 asks if i have been to the doctor for treatment. Well as far as my neck surgeon, no i havent been to see him in awhile because he discharged me and said that there was nothing further he could do for me and that when the other discs in my neck get worse to come back and see him. On my last review he was actually the review doctor, because i asked ss to let my own doctor do it. He put in a great report about my condition.
Well since i dont see him anymore does this hurt my case with ss? A freind of mine today said, alot of people who get ssdi probably never go back to see their surgeons after they are done treating them. I just want to be sure thats true.
Now as far as other doctors i see my regular doctor all the time and i have visits with my psychiatrist who treats my mental problems {3 differnt meds} and also does pain management (more meds) for me every 2-3 months.
I also see my psychotherapist once a month. Iam going to list these visits on question 5, but was wondering since it wasnt my neck surgeon that i was seeing for the recent doctor visits, does this hurt me.
Question 4 asks if my health is better, same or worse. Well truthfully since my last review if you throw in more mental problems am having along with added diabetes and high blood pressure problems, my health is worse. But i dont want to check that box, if checking causes a full blown cdr. Not that am worried, i just dont want to go thru that whole process again if i can help it. I will just check same, if thats a better answer.
And my last question is at the bottom of the sheet there is a spot for remarks.
I wanted to just add the names of the doctors am seeing, since in question they just ask what you went to the doctor for and not who the doctors are.
I also wanted to add that i also have a appointment at the end of the month with my neck surgeon,{due to my neck becoming a little worse} and figured i would also write a message saying how he released me and thats why there hasnt been any recent visits to him.
Does adding these comments trigger the whole questionaire to get thrown out and sent for a full review? Cause i had read that this form is just scanned by a machine, so i dont want to fill it out wrong and cost me going thru the headache of the full review again, already have enough stress.
I know this was long, but i wanted to make sure i explained it good enough, so that somebody can help me with these questions. I have been a nervous wreck since opening the mail today, even thou i knew this day was coming again.
And by the way my age is 44 and i was approved on my first try without any lawyer.

Hi Bobby and welcome to NeuroTalk. This is not BrainTalk. Totally different forum with a lot of the same people.

I'm not equipped to answer your questions, but I bet there will be somebody along very soon that can answer them.

Hi Bobby,
Now I'm on SSDI too, and have been for 8 to 10 years. You don't have to have all he disabilities in the world. You just have to have one and the doctors message that he doesn't want you working right now to have a strong case to stay in it. Now SS sends the message to my neurologist to get her message on if I'm still disabled. But at first I had to go to the SSA office to see their doctor to get checked out. So if you can get your neck specialist and your general practice doctor to both send SSDI a note on how you're still not doing good enough to get back out in the working world. Then SSDI gets hit hard on keeping you.
But it won't surprise me if SSA is doing all they can to kick people out of disability coverage. That's because a couple years ago bush told SS to no longer accept people like they were into SSDI because it would hurt his war budget over the years. Now when they say you have the right to a hearing, that sounds like court; but it's not. All you get is the talk with a high ranking supervisor in SS. I heard that on the news too when they reported what bush ordered SSA to do. But since you're already in it, and hearing from both doctors. That should hit them hard enough on having to keep you. But I'm not a doctor, that's just my opinion because I'm in it too because of severe epilepsy for 16 years.

Default CDR Review

Hi Bobby: This is probably a little late for the information that you need but I will give you the scenario I just completed. I got the short form on Jun 21 and mailed in it Jun 22. I answered quetion one that I was working, and that I was still working to this date, however I never made over the SGA limt and I have completed my trial work period. Question 2 I checked No for the attendance of any school or training. Question 3 My Doctor told me I could work, he went on to say only part time. Question 4...Condition remaning the Same. Question 5..Mark what you are being treated for and the medicines you are taking and any apppointments you have had receently with your doctor, no matter for what reason. Question 6: Self Explanatory. Remarks: Leave blank as you do not need to give them any additional information. You have the short form to fill out and the chances of becoming reviewed from the extremely intence reviews of have done, you will be all set. I heard backk from them in two months with no change. Plus your age could be in your favor, depending on what that may bee. Best of Luck!!!! Do not stress it is out of your control for now,, but to me you would be in good shape...

I just recieved my first CDR review form in the mail. I got the short form 6 question mailer. I filled it out and sent it back in. I have a question though, when I filled out question #5 I checked off no that I haven't seen a doctor recently for my disability. The only reason I answered no is because I am not going to see my doctor until August 22nd of 2012. I recieved the short form mailer in the mail on August 10th 2012. Should I be worried about my form getting flagged? I have been blind since birth and have been recieving SSI benefits since 2005. Any reply back would be appreciated.

I doubt it very much that you will be flagged just for that. They gotta know you didnt suddenly get your sight back.

Thank you. I hope it doesn't. Someone replied to my post and said that I should contact the social security office and tell them that I have an appointment on August 22nd just to be sure it didn't get flagged. Do you think I should call the social security office or do you think I should just wait and see what happens?

Default CDR Short form

I am stressing over this CDR review process. I recieved the short form 6 question form in the mail recently. I filled it out and mailed it and realized after I mailed it that I should have put on the form somewhere that I was going to see my doctor for my disability next week. Now I am worried that it is going to be flagged when they scan my form. My profile score is really low and I have been blind since birth. What are the chances that my form is going to be kicked out? The is the first time that I filled out the short CDR form so I don't know what to expect. I can't wait until this CDR process is over.
