Default freaking out!!!

oh i can't even seem to post correctly and i am soooo scared i had my psychologist help me fill out my update report...the 6 question ssdi form, and we corrected the phone number on the front page and now it will get kicked out and go to a person and then i am going to get the long form aren't i? i want to cry ....cognitive disorders do not help

I am a little confused with the reccomendations involving the 6 question form. I seen where it was reccomended that you state your last doctor visit as being for your original diagnosis. My question is this should I put my last visit date as the one for this on the form which would be over 6 months ago or should I put my last visit as the one earlier this month but for a different reason?

it is recommended not to fill out the remarks section, however some people who posted here did and theirs went through ok. you could also request another short form to complete. i also just requested a proof of benefits letter a few weeks before i got the short form....coincidence?

Default Requesting another form???

I just got my first request form and of course I jumped in when I received the Update Report (short form) and completed it before finding this helpful thread. I added remarks stating that my conditions have worsened and included a new diagnosis. I have NOT sent the form yet and would like to redo the form and want to request a new one. My question is, if I call to request another form, will that cause a 'red flag' and what reason should I give for asking for another form? Lost original, misplaced, etc? Also, concerning question 4 (describe your health), should I check same or worse? Thanks in advance and hope to hear back soon.

You should have no problem getting another form and am sure people do that on a daily basis. Theres a million reason why you could need a new form, all of which am sure SS doesnt care.

When you get the new form, you should make a bunch of blank copies thou, so when a mistake happens, you have another blank form

Actually, no they generally won't just send out another form if you call. You can use whiteout and hope that does the trick, or you can not return the form and they'll send out another.

Personally, I'd return the one you've filled out. Did you fill it out in the most ideal way? No. That doesn't mean you'll receive the Long Form though. And even those that fill out the short form in a way that causes no Long Form triggers, 5% still receive it.

Thank you for the update, I haven't called yet. If I use white out, should I then make a clean copy and send it in rather than the one with whiteout on it?
