Original is probably better. The only person I've heard that has received a "replacement" was a generic form without the beneficiary's barcode and personal info which is needed to scan.

you might be better off just sending in what you have already done rather than whiting it out. there have been people here who have written in the comments section and not received a long form. it might make it worse to send in a whited out form that will kick out of an optical reader.

Echoes, that was what I suggested, but I gave options.

Comments will likely be kicked to a human, but that doesn't mean an automatic Long Form, many will be fine. Whiteout may or may not be kicked to a human, but may look suspicious to that human.

if you are better put better, if you are worse put worse. if its been 3 years since you have been approved chances are you wont be the same. not many people stay the same, they either get better because the illness or condition is healing or worse because it has progressed. if its only been a year then you may well be the same.
people here give their own time to help other people who have questions about the process. no one is going to tell someone to fill something out a certain way to get approved if its not true.
