With proper documentation and a "worst-day scenario" letter from my psychologist, I was approved for SSDI for BD-II in two months (back in 2002).

Default CDR short form question

I recieved the CDR short form 6 question mailer in the mail on August 10th 2012. I filled it out and mailed it in right away. I have a question though. When I filled out question #5 I checked off NO that I haven't been to a doctor recently for my disability. The only reason I answered it like that is because I am not going to see my doctor for my disability until August 22nd 2012. Should I be worried that my short form is going to get flagged? I have been blind since birth and have been recieving ssi benefits since 2005. Any reply would be appreciated. Thanks in advance for answering this question for me.

I think Maybe call the office to make sure this isn't going to flag you. I think I would have written a note right on the form saying my appointment is Aug. 22nd. You don't want the agency to think you are not paying attention to your disability. I have had a few questions like that during my process, that I just enclosed a note right on the form to clearify things. I hope all works out for you. ginnie

If I call the social security office do I call my local social security office or do I call the place that I mailed the short form too? I should have just held on to the form and mailed it in after I went to my appointment. Thanks for replying so quickly, I really appreciate it.

Pawl,

Your specific question(s) tend to get more attention when you start your own thread. Newton's issue was all settled last year.

I agree with Ginnie, I would have noted right on the form that my next appt was August 22. If I was seeing my doc at least annually, I would have checked off "yes" to answer the questions if I saw a doctor recently for my disability.

Hi, another thing you can do, is keep a copy of all communication between you and the SS office. That way you can send in over again the same form corrected with your private note on it. Send a note to each of the places to cover your butt. I wish you all the best. ginnie
